On brain perfusion SPECT, a primary brain lesion presents as a localized defect that corresponds to the mass lesion. (99m)Tc-HMPAO images generally show a focal defect in the region of abnormality, whether containing necrotic tissue, recurrent tumor, or both. Further characterization with MR imaging is needed to confirm the diagnosis, as demonstrated in this case report.

Pseudomeningoceles are among the most common postoperative neurosurgical complications, usually presenting in the early postoperative period and often responding well to nonsurgical management. Here, we present a case of a giant cranial pseudomeningocele that developed three years after parasagittal meningioma resection, without any known risk factors. Despite conservative measures, the pseudomeningocele grew significantly over two years, reaching 22 cm along its long axis.

Background Prior to using the exoscope, we speculated that it represented an intermediate tool between a loupe and a microscope and had concerns about its visibility of deep, fine structures. Objective To evaluate the depths of meningioma for which the exoscope was suitable, and to clarify its disadvantages in meningioma resection. Methods Findings of consecutive meningioma surgeries using a 4K three-dimensional (3D) exoscope over a one-year period were evaluated for visibility of the surgical field, comfort of the surgeon's arm posture, the surgeon's head orientation, and perception of the image delay, accounting for the depth of the tumor.
