Although patients with heart disease have successfully completed marathon runs, the immediate cardiac effects of similar and greater distance endurance exercise competition are unknown. Two such cases are presented, demonstrating that vigorous exercise and extreme levels of fitness are not precluded in the cardiac patient.

To define training zones, ventilatory thresholds (VTs) are commonly established by cardiopulmonary gas-exchange analysis during incremental exercise tests. Portable near-infrared spectroscopy (NIRS) devices have emerged as a potential tool for detecting these thresholds by monitoring muscle oxygenation. This study evaluated the accuracy of NIRS measurements to determine VTs or critical power (CP) based on muscle oxygen saturation and assesses the device's consistency across 2 constant-load tests.

Objective: To explore the feasibility of post-exercise heart rate recovery indicators for predicting maximum oxygen uptake (VO2max) in healthy adults aged 30-60 years.
Methods: 260 healthy adults who did not perform regular exercise were randomly recruited and divided into a model group (n = 200) and a verification group (n = 60). Measure body fat percentage, weight, height and other indicators, and complete a cardiopulmonary exercise test as required to measure VO2max and heart rate recovery (HRR1, HRR2) in the first and second minutes after exercise.

This essay summarizes and integrates my experiences and observations-starting in the middle 1970s-as an athlete, scientist interested in human performance, biomedical researcher, and "expert," who sometimes advises athletes, coaches, and sports policy-makers. In this context, my focus has been primarily on endurance sports and five concepts underpin what I have learned over the last 50 years. (1) The "competitive significance principle" whereby athletes, coaches, and policy-makers are frequently interested in performance improvements of 1% or less.

Physical activity (PA) and exercise elicit adaptations and physiological responses in skeletal muscle, which are advantageous for preserving health and minimizing chronic illnesses. The complicated atmosphere of the exercise response can be attributed to hereditary and environmental variables.
