Background/purpose: Bloody nipple discharge (BND) is rare, distressing for parents, and presents a challenge for physicians.

Methods: We used PubMed to search for cases of BND that were diagnosed before adolescence and added data from two of our cases.

Results: The analyzed cohort comprised 46 patients (28 boys and 18 girls; mean [SD] age, 12.5±13.3months; range, 20days to 4years). The mean time for spontaneous resolution was 2.8±2.4months (range, 1week to 8months) after onset of BND without any intervention. The diagnosis was mammary ductal ectasia (MDE) in 15 patients, gynecomastia with MDE in two patients, hemorrhagic cysts in two patients, and gynecomastia alone in one patient. The majority (89.3%) of patients <1year old were managed conservatively, but half of them aged >1year (50.0%) underwent surgery. Surgery was performed more often in patients in whom a mass had been identified.

Conclusions: Age and findings at physical examination affect selection of treatment, but not sex. We found no reported cases of malignancy. Symptoms in children who are managed conservatively resolve within 10months. Children with BND should be conservatively managed to avoid the risk of developing breast deformities before adolescence.

With the popularity of breast screening, more and more intraductal papillary lesions (IDPL)were detected by ultrasound or mammography, which is a spectrum of diseases ranging from benign to malignant lesions. Patients with pathological nipple discharge (PND) may indicate IDPL, but it is difficult to clarify their biological characteristics before the operation. As to patients with IDPL without PND, it is even an enormous challenge to differentiate their benign or malignant characteristics.

Purpose: Pathological nipple discharge (PND) is associated with malignancy. This study aimed to investigate the value of fiberoptic ductoscopy (FDS) and the feasibility of immediate injection of methylene blue after FDS to identify discharging ducts and intraductal lesions without overflow of methylene blue during surgery.

Methods: From May 2019 to December 2023, 164 PND patients were enrolled.

Article Synopsis
  • A 73-year-old woman experienced bloody discharge and skin thickening in her right breast, prompting concerns for Paget disease.
  • A skin punch biopsy revealed high-grade infiltrating carcinoma with squamous features, ruling out other types like metaplastic carcinoma due to her medical history.
  • A lumpectomy confirmed invasive squamous cell carcinoma of the nipple, linked to Bowen disease, emphasizing the need for awareness of rare diagnoses in patients with prior breast cancer.

Giant cell tumor of soft tissue (GCT-ST) is an extremely rare phenomenon in the breast. Herein, a case involving a 75-year-old female with a painless lump and bloody discharge from the nipple of her left breast is reported. A diagnosis of malignant tumor was arrived at by observing the location of the tumor, interior echo, margins, vascular distribution, hardness, and microvascular density on preoperative multimodal ultrasonography.

Article Synopsis
  • A case study of a 34-year-old woman showed that ultrasound imaging played a key role in identifying NA when she experienced bloody discharge and nipple erosion.
  • Post-surgery analysis confirmed the benign nature of NA, highlighting the importance of using targeted ultrasound techniques for accurate diagnosis to prevent unnecessary invasive treatments.
