Both postural orthostatic tachycardia syndrome (POTS) and Long COVID are currently viewed as heterogeneous disorders with complex, multi-factorial and multi-systemic pathophysiology. Type 2 Diabetes is a highly prevalent chronic disorder that affects multiple systems through microvascular complications. Complications such as diabetic peripheral neuropathy, diabetic retinopathy, and diabetic vestibular dysfunction (vestibulopathy) all directly interfere with the sensory components of balance and postural stability. The resulting impairments cause increased falls risk and instability, making it difficult to perform daily task or exercise.

Background: Genu varum, or bow-legged deformity, involves an outward curvature of the lower limbs, often creating a visible gap between the knees when the ankles are together. Traditionally treated with invasive surgical methods like high tibial osteotomy, genu varum correction now includes less invasive options for patients with mild to moderate deformities. This commentary explores a novel approach using hyaluronic acid (HA) filler to volumise the medial gastrocnemius muscle, thereby improving calf contour and stability without surgery.

Clinical Vignette: A 23-year-old woman with pantothenate kinase-associated neurodegeneration (PKAN) presented with medication-refractory generalized dystonia and an associated gait impairment.
Clinical Dilemma: Bilateral globus pallidus internus (GPi) deep brain stimulation (DBS) can be an effective treatment for dystonia. However, outcomes for PKAN DBS have been variable and there are no standardized criteria for patient selection.
