A neurolinguistic and cerebral computed tomography (CT) study was carried out in 60 patients with aphasia. Fourteen had predominant subcortical involvement. Six of these showed involvement of the basal ganglia (5 with thalamic involvement), with small mass effect and small volume (less than 5 ml). The type of aphasia in this group was not uniform; it was remarkable, however, that initially there was mutism or initially non fluid language which soon became fluid in one half of the patients. Repetition and partially or totally preserved comprehension, together with dysarthic, dysprosodic and hypophonic abnormalities, were very common. The latter even had a more prolonged course than aphasic abnormalities. All the lesions in the remaining 8 patients had greater volume and were paracapsular in topography; 5 patients behaved as global aphasia (with a mean calculated volume of 23.7 ml), and the remaining 3 showed fluid aphasia, with an intermediate size (15 ml).

Cerebral arteriovenous malformations (AVMs) are rare but complex vascular anomalies, particularly challenging when located in eloquent regions such as the corpus callosum and post-central gyrus. This report aims to highlight the management and outcomes of a 41-year-old female patient with a hemorrhagic AVM in these critical areas, emphasizing the importance of early surgical intervention and advanced imaging techniques. The patient presented with a right-sided tonic-clonic seizure and expressive aphasia, prompting imaging that revealed a complex AVM with deep venous drainage and arterial supply from the anterior cerebral artery.

Introduction Parenteral heparin is widely used as bridging therapy while optimising oral anticoagulation(OAC). Newer Direct-Acting OACs(DOACs) attain therapeutic effect very quickly. We report the use of dabigatran as bridging therapy during warfarin optimization for cardioembolic stroke in two patients who opted to receive warfarin for long-term anticoagulation for secondary stroke prevention.

Clinical Reasoning: A 70-Year-Old Man With Systemic Illness Related Strokes Refractory to Medical Treatment Managed With Intracranial Stent.

Neurology

January 2025

From the Department of Medicine (L.P., V.B., M.C.F.S., R.F.), Pathology and Laboratory Medicine (J.P.M.), Division of Neurosurgery (B.D.), and Department of Radiology (INR and DNR) (B.D., R.F.), The Ottawa Hospital Research Institute, Ottawa, Ontario, Canada.

We present the case of a 70-year-old man with a history of embolic stroke, atrial fibrillation, deep vein thrombosis, and polymyalgia rheumatica who presented as a stroke code with transient right-sided focal neurologic deficits (motor and sensory), mild alteration in consciousness, and mild aphasia. His cerebrovascular imaging revealed new multifocal intracranial stenoses. Despite best medical management, this patient continued to have recurrent symptomatic cerebrovascular events.

Delayed cyst formation after radiosurgery for arteriovenous malformation: A case report and critical review.

Surg Neurol Int

October 2024

Neurosurgery Department, Unidade Local de Saúde de São João (ULS São João), Oporto, Portugal.

Background: Stereotactic radiosurgery (SRS) is a validated treatment option for cerebral arteriovenous malformations (AVMs), even if a greater knowledge of its potential delayed complications is still being acquired.

Case Description: A 49-year-old man suffered multiple episodes of cerebral hemorrhage in an approximate 10-year follow-up interval in the context of a left central core AVM with deep venous drainage into the internal cerebral veins (Spetzler Martin Grade 4) despite being treated with gamma knife radiosurgery at two separate timepoints, and with an almost complete obliteration confirmed. Approximately 10 years after the first radiosurgery treatment, he developed severe motor aphasia, Grade 3 right hemiparesis, progressive confusion, and memory deficits.

Purpose: As artificial intelligence (AI) takes an increasingly prominent role in health care, a growing body of research is being dedicated to its application in the investigation of communication sciences and disorders (CSD). This study aims to provide a comprehensive overview, serving as a valuable resource for researchers, developers, and professionals seeking to comprehend the evolving landscape of AI in CSD research.

Method: We conducted a bibliometric analysis of AI-based research in the discipline of CSD published up to December 2023.
