As health care payment reform continues to evolve, reimbursement increasingly is being linked to outcomes as well as to expenditures. Toward this end, the Centers for Medicare & Medicaid Services has established models for "bundled" payments to long-term care providers, using predetermined payments based on historical spending rates, in a new pay-for-performance landscape. The goal is to reward providers for quality and cost-effective care as well as penalize them for adverse patient outcomes and hospital readmissions based on the target spending rates. Pharmacists have a role in these new models of care, but need to broaden their partnerships and relationships with providers and be prepared to prove they are contributing both to quality care and to reducing costs.

Infectious microbes can spread rapidly from fomites (contaminated surfaces) via hand touch, with prolonged residence time on surfaces increasing transmission risk by extending exposure periods and/or involving more susceptible individuals. Existing studies have focused on decreasing microbial contamination, but not on the need for rapid removal from surface systems. This study introduces residence time as the time that a microbe spends within the surface system.

Individuals avoid spending cognitive effort unless expected rewards offset the perceived costs. Recent work employing tasks that provide explicit information about demands and incentives, suggests causal involvement of the Frontopolar Cortex (FPC) in effort-based decision-making. Using transcranial direct current stimulation (tDCS), we examined whether the FPC's role in motivating effort generalizes to sequential choice problems in which task demand and reward rates vary indirectly and as a function of experience.
