Objective: To report two cases of emphysematous cystitis, a rare, potentially serious disease.
Methods: Analysis of two different cases treated in our center and review of the existing literature.
Results: One patient underwent emergency surgery (radical cystectomy) due to the advanced stage of the disease. The second patient, whose was in an initial stage, benefited from a new treatment, consisting of hyperbaric oxygen and wide spectrum antibiotics.
Conclusions: Early diagnosis is the cornerstone of the conservative management of the disease. Hyperbaric oxygen therapy may be beneficial due to the improvement in oxygenation of the tissues affected by the disease.

This disease arises from mutations in the SLC2A1 and presents with a broad clinical spectrum. The enzyme dihydropyrimidine dehydrogenase (DPD) is the primary catabolic pathway of fluoropyrimidines including 5 fluorouracil (5FU) and capecitabine. Cases of lethal toxicity have been reported in cancer patients with complete DPD deficiency receiving standard dose of 5FU or capecitabine. DPD is encoded by the pharmacogene DPYD in which more than 200 variants have been identified.

This study presents two cases of central sleep apnea syndrome in children, highlighting the utility of assessing ventilatory control stability, particularly loop gain and central chemosensitivity in treatment decision-making. In the first case, elevated loop gain for oxygen correlated with periodic breathing, leading to successful treatment with supplemental oxygen in a 13 year-old boy with Prader-Willi-like syndrome. Conversely, in the second case, dealing with a 10 year-old girl with tumor in the brainstem-spinal cord junction, reduced loop gain prompted treatment with nocturnal non-invasive ventilation.
