Endothelial Keratoplasty (EK) is a corneal surgical procedure that allows a selective transplantation of the posterior layer of the cornea. Descemet's Membrane Endothelial Keratoplasty (DMEK) is one of the EK procedures in which the diseased Descemet's Membrane and the endothelium are replaced with a healthy donor tissue. To achieve this, the donor cornea is cut superficially from the endothelial side and the tissue can be separated using specific instruments like Pierse Notched, Acute or Fogla forceps. However, the pressure required to punch the superficial layer has always been a challenge and therefore a calibrated device to punch and excise the required superficial layer has been designed. This new model of punch will help to identify the peripheral edge of the DMEK lenticule which in turn helps to excise the tissue exclusively, further reducing the donor tissue wastage, as seen with the current tissue preparation methods.

Case Presentation: A 32-year-old male with a history of left eye keratoconus presented to the emergency department with left eye pain and blurry vision for two days. Out of concern for corneal hydrops, ophthalmology was consulted, and the diagnosis was confirmed. Per ophthalmology recommendations, the patient was started on hypertonic saline and prednisolone eye drops and referred to a corneal specialist.

Purpose: Alport syndrome (AS) is a genetic condition caused by a dysfunctional collagen (IV) α3α4α5 heterotrimer, leading to basement membrane instability and, ultimately, abnormalities in the kidney, inner ear, and eyes. This study aimed to characterize ocular pathology of AS by focusing on inflammatory and fibrotic markers.
Methods: Col4a3tm1Dec knockout (KO) mice eyes were evaluated for the localization of collagen (IV) α3 and collagen (IV) α4, then stained for transforming growth factor-β1 (TGF-β1), TGF-β2, connective tissue growth factor (CTGF), and β-catenin.
Urrets-Zavalia syndrome (UZS) stands out as a rare yet clinically consequential complication associated with ophthalmic surgery, marked by the emergence of a fixed and dilated pupil. While it was originally described post-penetrating keratoplasty (PKP) in patients with keratoconus and its prevalence is notably higher following PKP, instances of UZS have been documented in the context of other procedures, including intraocular lens implantation, Descemet-stripping endothelial keratoplasty and Descemet membrane endothelial keratoplasty (DMEK). We report a unique case of bilateral UZS after uneventful DMEK surgeries, highlighting a rare but potential complication after DMEK.
