Palmoplantar lichen planus is a localized and rare subtype of lichen planus (LP) often underdiagnosed. Several morphological types of palmoplantar lesions have been defined in LP. We present an unusual case of the palmoplantar kyperkeratotic variant of LP. Histopathology examination confirmed our diagnosis. We emphasize the importance of this rare entity in the differential diagnosis of palmoplantar dermatoses.

We present the case of a 42-year-old Indian male with prurigo nodularis, exhibiting multiple verrucous, brownish-black nodules on the left lower and right upper extremities, with milder involvement of the right lower extremity. The lesions were asymmetrically distributed in a near-linear pattern, with prominent white hyperkeratosis and associated moderate pruritus and paresthesia. Differential diagnoses included pemphigoid nodularis, verruca vulgaris, and hypertrophic lichen planus, with a biopsy confirming prurigo nodularis.

Our study aimed to establish the basic reliability parameters of direct immunofluorescence test results in patients with oral lichen planus. We conducted an evaluation of individual antibody classes in the DIF and ELISA (BP180 antigen), comparing these results with the classical histopathological (HP) examination in a group of patients treated within the standard healthcare in our clinic. Among 66 participants with oral changes indicative of LP, only 50% received histopathological confirmation of the LP diagnosis.
