The association between varying durations of previous vancomycin therapy and subsequent daptomycin success rates was evaluated in a subset of patients with methicillin-resistant Staphylococcus aureus bacteremia from the Cubicin® Outcomes Registry and Experience 2005-2009 (n=159). Previous vancomycin exposure did not adversely affect daptomycin outcomes; success rates were 81% in patients treated with first-line daptomycin and 89%, 83%, and 88%, respectively, in patients with 1-3days, 4-6days, and ≥7days of previous vancomycin exposure (P=0.5).

Enteral administration of vancomycin is the standard treatment for () colitis and is presumed to have no systemic absorption. In critically ill patients, however, especially with multi-organ failure, enteral absorption of vancomycin is unpredictable and can cause severe toxicity if it remains unrecognized. We therefore report a case of systemic absorption of enteric vancomycin in a patient with severe colitis.

Bilateral preseptal cellulitis without accompanying sinusitis or skin trauma is uncommon. In this report, we present a case of bilateral preseptal cellulitis and an upper eyelid abscess in an otherwise healthy child. A nine-year-old girl presented with severe and progressive bilateral swelling of the upper lids that showed an unsatisfactory response to medical treatments (intravenous ceftazidime and vancomycin) and warranted a referral to our facility.

Background And Objective: The latest consensus recommends using the ratio between the area under the curve over 24 h (AUC) and minimal inhibitory concentration (MIC) as the therapeutic target for vancomycin in clinical practice, with a Bayesian approach and population pharmacokinetic (popPK) model being particularly recommended. While using both post-dose peak concentration (C) and pre-dose concentration (C) is more accurate than C alone, the optimal sampling strategy for estimating AUC is still unclear. The objective of this study was to determine the best sampling time(s) to estimate AUC using the Bayesian approach in these specific adult hematologic cancer patients.

Population pharmacokinetic (popPK) models are an essential tool when implementing therapeutic drug monitoring (TDM) and to overcome dosing challenges in neonates in clinical practice. Since vancomycin, gentamicin, and amikacin are among the most prescribed antibiotics for the neonatal population, we aimed to characterize the popPK models of these antibiotics and the covariates that may influence the pharmacokinetic parameters in neonates and infants with no previous pathologies.
