AI Article Synopsis

  • Atrial fibrillation (AF) significantly increases the risk of developing dementia, with a higher annual incidence of dementia observed in AF patients compared to non-AF subjects.
  • The study analyzed data from over 332,000 AF patients in Taiwan, comparing them to matched controls without AF or dementia, and found that AF patients had a hazard ratio of 1.420 for developing dementia after accounting for various factors.
  • The CHA2DS2-VASc score was identified as a better predictor for dementia risk in AF patients than the CHADS2 score, indicating its usefulness for assessing dementia risk among this population.

Article Abstract

Background: Atrial fibrillation (AF) is associated with an increased risk of cognitive impairment and functional decline, and may contribute to development of dementia.

Objectives: Data from a nationwide large-scale population-based cohort study are lacking. Besides, how best to predict the occurrence of incident dementia among AF subjects remains uncertain.

Methods: A total of 332,665 AF subjects without dementia were identified as the study group from the "National Health Insurance Research Database" in Taiwan. For each study patient, one age- and sex-matched subject without AF and dementia was selected as the control group. The study end point was occurrence of dementia, and the usefulness of CHADS2 and CHA2DS2-VASc scores in predicting dementia was analyzed.

Results: During the follow-up, 29,012 AF patients experienced dementia with an annual incidence of 2.12%, higher than non-AF subjects (1.50%). Patients with AF possessed a higher risk of dementia with a hazard ratio (HR) of 1.420 after adjustments for age, gender, baseline differences and medication use. Among AF patients, the CHADS2 and CHA2DS2-VASc scores were significant predictors of dementia with an adjusted HR of 1.520 and 1.497 per 1 increment of the CHADS2 and CHA2DS2-VASc scores, respectively. The c-index for CHA2DS2-VASc in predicting dementia (0.611, 95% confidence interval [CI]=0.608-0.614) was significantly higher than the CHADS2 score (0.589, 95% CI=0.586-0.592) (DeLong test p<0.001).

Conclusions: In this nationwide cohort study, AF was independently associated with a higher risk of dementia. The CHA2DS2-VASc score can be used to estimate the risk of dementia in AF patients.

Background: Patients with non-valvular atrial fibrillation (nvAF) who experienced a cardioembolic (CE) event despite adequate oral anticoagulation (OAC) are at high risk of recurrence and the combination between percutaneous left atrial appendage closure (LAAC) and long-term OAC may be a valuable option. The aim of this study was to compare the safety and the efficacy of post-LAAC long-term assumption of direct oral anticoagulants (DOACs) vs. vitamin K antagonists (VKAs) in this population.

Background: The Amulet IDE trial (AMPLATZER Amulet Left Atrial Appendage Occluder [LAAO] Investigational Device Exemption [IDE] Trial) evaluated the safety and effectiveness of the Amulet occluder (Abbott) in patients with nonvalvular atrial fibrillation. The Amulet IDE trial is the largest randomized LAAO trial, comparing the Amulet occluder with the Watchman 2.5 device (Boston Scientific).

Article Synopsis
  • * The study compares a new treatment technique, high-power short-duration pulmonary vein isolation (HPSD-PVI), with traditional methods to see which is more effective in preventing recurrent atrial tachyarrhythmia (ATA).
  • * Analyzing 969 patients, the research found that different factors predicted recurrence for each technique, with the HPSD-PVI group showing a trend towards lower recurrence rates compared to the conventional method.

Department of Biostatistics and Medical Informatics, University of Wisconsin-Madison, Madison, WI 53726, United States.

Objective: This study aims to develop machine learning models that provide both accurate and equitable predictions of 2-year stroke risk for patients with atrial fibrillation across diverse racial groups.

Materials And Methods: Our study utilized structured electronic health records (EHR) data from the All of Us Research Program. Machine learning models (LightGBM) were utilized to capture the relations between stroke risks and the predictors used by the widely recognized CHADS2 and CHA2DS2-VASc scores.

Background: Atrial fibrillation after cardiac surgery (POAF) is associated with increased morbidity and mortality. Several scores were used to predict POAF, with variable results. Thus, this study assessed the performance of several scoring systems to predict POAF after mitral valve surgery.
