Encephalopathy associated with metronidazole administration is an uncommon but potentially reversible disease and depends on the cumulative metronidazole dose, and most patients with this condition recover rapidly after discontinuation of therapy. We present a case as well as a review of the literature regarding this rare but serious adverse event.

We report a rare case of concurrent onset of osimertinib-induced heart failure and metronidazole-induced encephalopathy during treatment of a brain abscess. A 78-year-old female with lung adenocarcinoma presented with neurological symptoms and was diagnosed with a brain abscess. During treatment, she developed heart failure and encephalopathy, linked to osimertinib and metronidazole, respectively.

Metronidazole-induced encephalopathy (MIE) is a rare toxic encephalopathy. We describe a reversible MIE case in a patient with hereditary hemorrhagic telangiectasia (HHT), treated with metronidazole for brain abscess, who developed dizziness, weakness, dysarthria, and severe dysmetria. His Magnetic Resonance Imaging (MRI) brain revealed bilateral, symmetric lesions in bilateral symmetrical regions of increased intensity in the medullary olives, cerebellar dentate nuclei, and the dorsal pons, all characteristic of MIE.
