The course of the fistula tract in relation to the anal sphincter is identified by clinical examination under general anaesthesia using a fistula probe and injection of fluid into the external fistula opening. In the event of a complex fistula or in the case of fistula recurrence, this should be supplemented with an endoluminal ultrasound scan and/or an MRI scan. St. Mark's fistula chart should be used for the description. Simple fistulas are amenable to fistulotomy, whereas treatment of complex fistulas requires special expertise and management of all available treatment modalities to tailor the right operation to the individual patient. The given levels of evidence and grades of recommendations are according to the Oxford Centre for Evidence-based Medicine (www.cemb.net).

The presence of an aberrant right hepatic artery (a-RHA) could influence the oncological and postoperative outcomes after pancreaticoduodenectomy (PD). A comparative study was conducted, including patients who underwent PD with a-RHA or with normal RHA anatomy. The primary endpoints were R1 resection in all margins (pancreatic, anterior, posterior, superior mesenteric artery, and portal groove), overall survival (OS), and disease-free survival (DFS).

Evolution of different forearm flap designs in phalloplasty.

Ann Chir Plast Esthet

January 2025

Service de Chirurgie Plastique, Réparatrice, Esthétique, Hôpital Cavale-Blanche, Brest, France.

Phalloplasty is one of the most complex procedures in plastic surgery. The criteria for an ideal neophallus include an aesthetic and erogenous phallus that allows for sexual intercourse, with a functional neo-urethra enabling standing urination. The free forearm flap has become the gold standard for phallic reconstruction.

Advanced Imaging of Type 2 Spinal CSF Leaks with Ultrahigh-Resolution Cone-Beam CT Myelography.

AJNR Am J Neuroradiol

January 2025

From Department of Neuroradiology (Niklas Lützen, Charlotte Zander, Horst Urbach), Medical Center - University of Freiburg, Faculty of Medicine, University of Freiburg, Germany and Department of Neurosurgery (Jürgen Beck, Florian Volz, Katharina Wolf, Amir El Rahal), Medical Center - University of Freiburg, Faculty of Medicine, University of Freiburg, Germany.

Type 2 CSF leaks are spinal lateral dural tears, causing spontaneous intracranial hypotension (SIH). They may be visualized with digital subtraction myelography (DSM), cone-beam CT (CBCT) myelography, energy-integrating detector or photon-counting CT myelography. A recently introduced ultrahigh-resolution cone-beam CT (UHR-CBCT) myelography has shown beneficial visualization of CSF-venous fistula, another cause of SIH.

Background And Purpose: Current evidence suggests that tentorial dural arteriovenous fistulas with endovascular treatment offer a high rate of occlusion and reduced procedural risks. Here we report the clinical and angiographic outcomes in patients with tentorial dural arteriovenous fistulas who underwent endovascular treatment as first-line treatment.

Materials And Methods: A retrospective analysis was conducted on 83 patients with tentorial dural arteriovenous fistulas treated at our center from April 2009 to November 2023 using endovascular treatment.

Pancreatic duct (PD) strictures, leaks, and disconnected ducts are important morphologic consequences of inflammatory disease of the pancreas, resulting in abdominal pain, pancreatic ascites, pancreatic pleural effusion, and external pancreatic fistula. Traditionally, these PD complications were treated surgically, but a better understanding of their pathophysiology, along with advancement in endoscopic interventions, has transformed the therapy from morbid surgical interventions to minimally invasive, safe, and effective endoscopic treatment. This review discusses the current diagnostic and management strategies for PD strictures, leaks, and disconnected pancreatic ducts.
