Urinary tract infections are the most commonly acquired bacterial infections, but the significance of growths from catheter-related specimens, particularly heavy mixed growths, is uncertain and can pose a dilemma for the clinical laboratory responsible for processing and authorising reports, as well as a diagnostic dilemma for clinicians. Furthermore, inappropriate processing and reporting of samples may lead to inappropriate treatment of patients, which can result in adverse effects as well as increased laboratory and clinical costs. This short communication summarises recent evidence and guidelines on the matter.

Objective: To evaluate the uterine microbiome among women with endometrial polyps and submucosal fibroids and to compare results between endometrial sampling techniques.
Design: Patients with polyps or fibroids were prospectively recruited prior to hysteroscopy, while patients undergoing retrieval for planned oocyte cryopreservation were recruited prospectively as controls. Three specimen types obtained for each patient were the distal 5 mm of an embryo catheter passed to the uterine fundus (C), endometrial tissue from an endometrial biopsy (T), and formalin fixed paraffin embedded endometrial tissue from the same endometrial biopsy (FFPE).

The role of initial specimen diversion devices (ISDDs) in preventing contamination of central venous catheter (CVC) blood cultures is undefined. A model to simulate CVC colonization and contamination compared standard cultures with ISDD technique. ISDD detected 100% of colonized CVCs while decreasing false-positive cultures from 36% to 16%.
