Laparoscopic donor nephrectomy was introduced in 1995 as a means of minimally invasive surgeries that entail kidney extraction from healthy individuals. Since then, it has widely overtaken the traditional open surgical approaches, especially in live donor nephrectomy procedures worldwide. Laparoendoscopic single-site surgery is considered a more optimized surgical approach utilizing a single incision instead of four.

Article Synopsis
  • Internal hernias are a rare but serious cause of small intestinal obstructions, accounting for 0.6-5.8% of cases, with a small percentage being congenital trans mesenteric types.
  • A case study of a 38-year-old male highlights the urgency of surgical intervention after presenting with severe abdominal symptoms, ultimately leading to a diagnosis of a strangulated trans mesenteric hernia.
  • Timely diagnosis and surgery are crucial to prevent severe complications like bowel gangrene; laparotomy is the recommended treatment to address these emergencies and prevent future occurrences.

Although internal hernias are uncommon, they must be beared in mind in the differential diagnosis in cases of intestinal obstruction, especially in patients with no history of previous surgery or trauma. Because of the high possibility of strangulation and ischemia of the affected loops, internal hernias represent a potentially life-threatening condition and surgical emergency that needs to be quickly recognized and managed promptly. Imaging plays a leading role in the diagnosis and in particular multidetector computed tomography (MDCT), with its thin-section and high-resolution multiplanar reformatted (MPR) images, represents the first line image technique in these patients.
