Enterococci that are nonsusceptible (NS; MIC > 4 μg/ml) to daptomycin are an emerging clinical concern. The synergistic combination of daptomycin plus beta-lactams has been shown to be effective against vancomycin-resistant Enterococcus (VRE) species in vitro. This study systematically evaluated by in vitro time-kill studies the effect of daptomycin in combination with ampicillin, cefazolin, ceftriaxone, ceftaroline, ertapenem, gentamicin, tigecycline, and rifampin, for a collection of 9 daptomycin-NS enterococci that exhibited a broad range of MICs and different resistance-conferring mutations. We found that ampicillin plus daptomycin yielded the most consistent synergy but did so only for isolates with mutations to the liaFSR system. Daptomycin binding was found to be enhanced by ampicillin in a representative isolate with such mutations but not for an isolate with mutation to the yycFGHIJ system. In contrast, ampicillin enhanced the killing of the LL-37 human antimicrobial peptide against daptomycin-NS E. faecium with either the liaFSR or yycFGHIJ mutation. Antagonism was noted only for rifampin and tigecycline and only for 2 or 3 isolates. These data add support to the growing body of evidence indicating that therapy combining daptomycin and ampicillin may be helpful in eradicating refractory VRE infections.

Metabolic engineering of Streptomyces roseosporus for increased production of clinically important antibiotic daptomycin.

Daptomycin (DAP), a novel cyclic lipopeptide antibiotic produced by Streptomyces roseosporus, is clinically important for treatment of infections caused by multidrug-resistant Gram-positive pathogens, but the low yield hampers its large-scale industrial production. Here, we describe a combination metabolic engineering strategy for constructing a DAP high-yielding strain. Initially, we enhanced aspartate (Asp) precursor supply in S.

Introduction: Infective endocarditis (IE) due to methicillin-resistant Staphylococcus aureus (MRSA) is characterized by frequent treatment failure to first-line agents and high mortality, necessitating use of alternative management strategies. Ceftaroline fosamil (CPT) is a cephalosporin antibiotic with activity against MRSA but without regulatory approval for the indication of IE. This study describes clinical experience with CPT-based regimens utilized in MRSA-IE.

Article Synopsis
  • Daptomycin (DAP) is an antibiotic used to treat drug-resistant infections like MRSA, but resistance to DAP is becoming more common and occurs through various mechanisms.
  • This study examines how DAP resistance affects the sensitivity to the phage Sb-1 in 14 clinical MRSA strains, finding that resistant strains are more susceptible to the phage.
  • Results show that DAP-resistant mutant strains have increased phage sensitivity and differences in their cell wall structure compared to DAP-susceptible strains, suggesting that using phages alongside DAP could be an effective way to combat resistance.*

Objective: We report our experience with the use of ceftaroline in combination with daptomycin or vancomycin for methicillin resistant coagulase negative bacteraemia.

Methods: A multicentre retrospective study was carried out at three institutions of adult patients with methicillin resistant or bacteraemia who were managed with either daptomycin or vancomycin in combination with ceftaroline.

Results: Twelve patients met the inclusion criteria.
