Introduction: This article presents a case of nail unit melanoma (NUM), highlighting the unusual clinical presentations as those originating from the hyponychium. We discuss how dermoscopy is an essential tool for this pigmented lesion, and how the lack of a standardized guide for NUM underscores the importance of personalized approaches to ensure the best possible outcomes.
Case Presentation: A 72-year-old woman presented with a melanocytic lesion on her right second finger, characterized by a hyperpigmented macule with irregular borders and a heterogeneous distribution of pigment.

Rare subtypes of cutaneous T-cell lymphomas (CTCL) include four entities, primary cutaneous γδ T-cell lymphoma, primary cutaneous CD8+ aggressive epidermotropic cytotoxic T-cell lymphoma, and primary cutaneous CD4+ small/medium T-cell lymphoproliferative disorders, primary cutaneous acral CD8+ T-cell lymphoma, which were previously considered provisional and are now included in the new 5 World Health Organization classification of hematolymphoid tumors as distinct entities. An updated summary of the clinical, histological, and genomic characteristics of these uncommon CTCL subtypes is given in this review, with a focus on the growing body of knowledge regarding their classification and possible treatment strategies.

Metastatic hand tumors are uncommon but important to include in the differential diagnosis for hand masses. In this study, we report the case of a patient presenting initially with hand pain and swelling with no other pertinent medical history except for an extensive smoking history. Subsequent mass biopsy and work-up revealed metastatic lung cancer.
