In order to develop a national allocation scheme for donor pancreases, factors affecting waiting time and transplant outcomes in the United States (US) and United Kingdom (UK) were analyzed and compared. Blood group, sensitization, dialysis requirement, and whether the patient was waiting for a kidney and pancreas or pancreas alone affected waiting time in both countries; ethnicity and body mass index (BMI) also affected waiting time in the US. Ninety-day pancreas survival was similar in the UK and US, and was poorer for patients receiving a pancreas alone, with older donors, higher BMI and longer duration of ischemia in both countries. Factors affecting outcome, together with published data on factors affecting islet transplantation, informed the development of a points based allocation scheme for deceased donor pancreases in the UK providing equitable access for both whole organ and islet recipients through a single waiting list. Analysis of the allocation scheme 3 years after its introduction in December 2010 showed that the results were broadly as simulated, with a significant reduction in the number of long waiting patients and an increase in the number of islet transplants. There remains a surplus of highly sensitized patients in the waiting list, which the scheme should address in time.

Universal Health Coverage (UHC) aims to provide access to quality health services to all while avoiding financial hardship. Strategies can include establishing a national health insurance scheme (NHIS). However, variations in the progress exist among countries with an NHIS.

Objectives: This study accessed the challenges with financing cardiothoracic surgical care in Africa, highlighting the present state of funding, and proffering probable solutions to adequate and effective funding in the region.
Methods: Through a literature review, the authors elaborate on key points such as; areas of financial funding in cardiothoracic surgery, barriers to appropriate allocation of financial resources for cardiothoracic surgery in Africa, and the needs and available resources for cardiothoracic surgery in Africa. Multiple search engines and databases were used, like: PubMed, Medline, Cochrane, Scopus, and Google Scholar.

In this article, we consider an UAV (unmanned aerial vehicle)-assisted free space optical (FSO) secure communication network. Since FSO signal is impossible to detect by eavesdroppers without proper beam alignment and security authentication, a BS employs FSO technique to transfer information to multiple authenticated sensors, to improve the transmission security and reliability with the help of an UAV relay with decode and forward (DF) mode. All the sensors need to first send information to the UAV to obtain security authentication, and then the UAV forwards corresponding information to them.

We develop a novel key routing algorithm for quantum key distribution (QKD) networks that utilizes a distribution of keys between remote nodes, i.e., not directly connected by a QKD link, through multiple non-overlapping paths.

In this work, we unveil the advantages of synergizing cooperative rate splitting (CRS) with user relaying and simultaneously transmitting and reflecting reconfigurable intelligent surface (STAR RIS). Specifically, we propose a novel STAR RIS-assisted CRS transmission framework, featuring six unique transmission modes that leverage various combinations of the relaying protocols (including full duplex-FD and half duplex-HD) and the STAR RIS configuration protocols (including energy splitting-ES, mode switching-MS, and time splitting-TS). With the objective of maximizing the minimum user rate, we then propose a unified successive convex approximation (SCA)-based alternative optimization (AO) algorithm to jointly optimize the transmit active beamforming, common rate allocation, STAR RIS passive beamforming, as well as time allocation (for HD or TS protocols) subject to the transmit power constraint at the base station (BS) and the law of energy conservation at the STAR RIS.
