Background: We aimed at assessing the efficacy of the patch plasty technique without endarterectomy in patients with diffuse coronary artery. Long anastomosis of the left internal mammary artery graft (LIMA) to the left anterior descending (LAD) artery was performed and examined using transthoracic Doppler echocardiography to detect coronary flow reserve (CFR) and epicardial stenosis.

Methods: Forty-one patients (6 women; mean age, 58 ± 9 years) who underwent coronary artery bypass surgery using the patch plasty technique without endarterectomy were included in the study. Presence of CFR was examined in each patient by transthoracic Doppler echocardiography.

Results: One of the patients (2.4%) died on the first postoperative day. The remaining patients were divided into 2 groups: those with normal CFR (CFR ≥ 2) (n = 35, 88%) and those with low CFR (CFR < 2) (n = 5, 12.0%). The length of patch plasty (3.6 ± 0.82 cm) in the low CFR group was significantly longer than that in the normal CFR group (2.69 ± 0.75 cm). Coronary angiography was performed for the 3 patients with CFR < 2: Two patients showed normal grafts and anastomoses, but the third patient's distal LAD-LIMA anastomosis was almost 90% occluded.

Conclusion: We elucidated the reliability of the patch plasty without endarterectomy method and transthoracic Doppler echocardiography for detecting the severity of coronary artery disease.

Rare but important gastrointestinal complications after laparoscopic inguinal hernia repair: a single-center experience.

Transabdominal preperitoneal patch plasty (TAPP) versus total extraperitoneal patch plasty (TEP) are surgical techniques commonly used to treat inguinal hernia. However, studies indicate that both procedures may lead to significant complications, particularly gastrointestinal complications, some of which can be life-threatening. We statistically analyzed the complications caused by adult inguinal hernia patients admitted from 2018 to 2022.

Article Synopsis
  • The study investigates the effectiveness of coronary artery endarterectomy during coronary bypass surgery, particularly focusing on the left anterior descending artery (LAD), and its impact on patient outcomes.
  • It involved a review of 20 patients who underwent this procedure between January 2014 and December 2021, assessing mid-term graft patency and overall survival rates.
  • Results indicated a 60% patency rate in grafts after an average follow-up of about 33 months, and a high survival rate of 95%, suggesting that endarterectomy can be a reliable option when suitable anastomosis sites are lacking.

Background: Pulmonary artery banding (PAB) is performed as a palliative surgery for congenital heart diseases. Although pulmonary stenosis is one of the complications of PAB, symptomatic supravalvular pulmonary stenosis (SVPS) in adulthood after PAB is an extremely rare condition. Further, very few studies have focused on surgical cases of SVPS in adult.

Introduction: Hernia repair is a common procedure performed by general surgeons. Introduced in 1990s, the use of laparoscopic hernia repair has recently increased and, consequently, rare complications previously unknown have been reported.

Presentation Of Case: A 43-years-old male patient who underwent a transabdominal preperitoneal patch plasty (TAPP) procedure for symptomatic bilateral inguinal hernia.
