We invented a new method for fixing an intraocular lens (IOL) in the scleral tunnel without using a wide conjunctival incision. Modified bent catheter needles were used to penetrate the IOL haptics through the sclerotomy sites. The IOL haptics were inserted into 30-guage (G) scleral tunnels guided by double 30-G needles piercing the sclera. All procedures were performed through the conjunctiva without wide incision. The procedure does not require special forceps, trocars or fibrin glue, only catheter and 30-G needles. The aid of an assistant was not required to support the IOL haptic. The procedures were easily learnt based on our previous method. As with other transconjunctival sutureless surgeries, patients feel less discomfort and the conjunctiva can be conserved for future glaucoma surgery. Complications included two cases of vitreous haemorrhage (16.7%), and one case each of postoperative hypotony, and iris capture (8.3%). Astigmatism induced by intraocular aberration was the same as we reported previously. Our method for fixing the IOL into the scleral tunnel is innovative, less expensive, less invasive and quick. This modified method is a good alternative for fixing IOL haptics into the sclera.

Intrascleral haptic fixation of intraocular lens (IOL) is an extremely useful technique to provide visual rehabilitation in eyes with inadequate capsular support. It requires exteriorization of haptics along with tucking of haptics in the scleral groove preferably and conventionally in the horizontal meridian. In eyes with large corneal diameter, there is difficulty in tucking enough length of the haptics into the intrascleral groove, carrying the risk of slippage of haptics and decentration of IOL.

Purpose: To study change in the contact of intraocular lens (IOL) with the posterior capsule with respect to the vertical versus horizontal orientation of the haptic-optic junction of the IOL using intraoperative spectral domain optical coherence tomography (SD-OCT).
Methods: Fifty eyes of 50 patients with senile immature cataract underwent topical phacoemulsification procedure with implantation of a monofocal IOL by a single experienced surgeon. The Rescan 700 SD-OCT system was used for intraoperative imaging.

Intraocular lens (IOL) dislocation is not an uncommon complication and often requires surgical intervention, depending on the status of capsular bag support. Conventionally, posterior dislocation of a foldable IOL or the IOL-bag complex warrants their removal as foldable IOLs are not ideal for sulcus placement. The technique presented here describes using quadrilateral sutures to refix looped haptic IOLs at the ciliary sulcus with or without a bag complex.

Scleral-fixated intraocular lens (SFIOL) is a widely used technique for IOL implantation in patients where capsular support is insufficient. Most surgeons have shifted away from sutured to sutureless SFIOL techniques where haptics of a multifocal IOL are inserted in scleral tunnels/flaps. Large-scale publications have shown wide variation in the refractive status of eyes post-SFIOL even in the best of the hands.
