This case report presents a three-year follow-up of a young male with bilateral hindfoot fractures due to a high-energy road traffic accident, resulting in a comminuted open calcaneal fracture on the left and an open fracture-dislocation of the right talus. Staged reconstruction was performed, including initial debridement, temporary cement spacers, and subsequent fixation with femoral head allografts (FHAs). The right foot underwent a tibiotalocalcaneal (TTC) fusion, and the left foot received a double arthrodesis.

Minimally invasive surgery is gaining tremendous popularity in reconstructive foot and ankle surgery, as well as with trauma. Minimally invasive approaches have demonstrated equivalent to outcomes to traditional open incisional approaches with the added benefit of less risk for wound healing complications and surgical site infections. Advances in orthopedic hardware and surgical techniques are allowing minimal incision surgery for trauma to become more widespread.

Tibiotalocalcaneal (TTC) fusion with an intramedullary nail (IMN) has been utilized for a myriad of indications in hindfoot and ankle reconstruction. However, some controversies remain on the optimal position of the hindfoot. Previous studies have reported on the potential medialization of the rearfoot during insertion of the IMN, but few studies have examined the potential affect on the subtalar joint.
