Objectives: Patients with a bicuspid aortic valve (BAV) present with various phenotypes of the valve, the aortic root (AoR) and/or the ascending aorta (AAo) and various valve haemodynamics (vHs). The aim of the present study was to investigate the association between the above parameters.

Methods: Between February 1999 and April 2014, the preoperative aortographies of 828 surgical patients with BAV were evaluated. The exact BAV type was classified intraoperatively according to the number and spatial orientation of the raphes.

Results: On analysis of BAV phenotypes and aortic configurations, a weak pattern was revealed (P = 0.01) only for BAV type 0 and AoR dilatation. Including haemodynamics, certain significant patterns emerged: in insufficient BAVs, AoR dilatation was significantly more frequent in type 0, type 1 LR and type 1 RN, whereas AoR + AAo dilatation was more frequent in BAV type 1 LR. In stenotic BAVs, AAo dilatation alone was observed significantly more frequently in BAV type 1 LR and type 0. Combined vHs were associated with AAo and AoR + AAo dilatation in BAV type 2/unicuspid only.

Conclusions: Associations between the two parameters, BAV type and aortic configuration, were only weak. With the inclusion of haemodynamics significant associations emerged but were not exclusive. In stenotic BAVs, aortic dilatation was more localized to AAo only, supporting the valve-related flow turbulence theory. Insufficient (even trace insufficient) BAVs and also combined lesions in BAV type 2/unicuspid were associated with a more extensive aortopathy (AoR + AAo) in younger patients, indicating a more aggressive gene-related aortopathy.

Transcatheter aortic valve replacement (TAVR) in patients with severe aortic stenosis and raphe-type bicuspid aortic valve (BAV) is still associated with poor outcomes in terms of increased risk of paravalvular regurgitation, stroke, and permanent pacemaker implantation. There is no definitive consensus on the optimal sizing method for prosthesis selection in this setting. The LIRA method is a supra-annular tailored sizing method specifically designed for bicuspid anatomy that might increase accuracy of prosthesis choice in BAV patients and improve TAVR outcomes.

Background: Transcatheter aortic valve replacement (TAVR) has evolved from a novel technology to an established therapy for high-risk patients with symptomatic severe aortic valve stenosis (AS). Recently, its use has also been extended to low-risk patients, resulting in its increasing utilization in patients with bicuspid aortic valve (BAV). But as a serious post-TAVR complication, ischemic stroke was associated with a nearly 6-fold increased 30-day mortality.

Background: Transcatheter aortic valve replacement (TAVR) in patients with bicuspid aortic valve (BAV) disease is still burdened by a non-negligible rate of stroke and permanent pacemaker implantation (PPI). These suboptimal results, possibly related to the unique BAV anatomy, may suggest the use of a different sizing method in this setting. The aim of our study is to evaluate whether the application of the supra-annular LIRA method may improve clinical outcomes in this population.

Objectives: The bicuspid aortic valve (BAV) is the most common congenital heart defect. Patients with BAV frequently develop aortopathy, which depends on the dysfunction and morphotype of the BAV.

Aim: The aim of our study was to compare the echocardiography and cardiac magnetic resonance (CMR) findings in BAV patients, and to define the risks of BAV dysfunction and aortopathy.

Chemical and Heat Treatment for Viral Inactivation in Porcine-Derived Gelatin.

BMC Biotechnol

December 2024

Biomedical Department, R&D Center, Nitta Gelatin Inc, 2-22, Futamata, Yao City, Osaka, 581-0024, Japan.

Article Synopsis
  • - The study focuses on evaluating the effectiveness of virus inactivation methods used in the manufacturing process of porcine-derived gelatin, which is commonly used in pharmaceuticals and medical devices.
  • - Researchers tested various model viruses and found that both chemical and heat treatments significantly reduced viral presence, achieving a log reduction value (LRV) greater than 4, indicating effective inactivation.
  • - The findings confirm that implementing these treatment steps in gelatin production enhances safety by minimizing potential viral contamination in biomedical applications.
