We present current concepts and evidence to optimize diagnosis and management of osteoarthritis in the thumb joints. Numerous options and controversies exist for surgical treatment of carpometacarpal joint arthritis. Fewer options exist for metacarpophalangeal joint arthritis. Surgical treatment for interphalangeal arthritis is mainly arthrodesis.

Osteoarthritis of the scaphotrapeziotrapezoidal joint is frequent but often pain-free or well tolerated with non-surgical treatment. Surgical options are numerous and none seems to have clear evidence of superiority. In addition to well-established procedures, such as scaphotrapeziotrapezoidal joint arthrodesis, distal scaphoid resection and trapeziectomy, more recent surgical techniques have been reported.

Objective: We present long-term results for the Pyrocardan®, a pyrocarbon trapeziometacarpal interposition implant used for the treatment of osteoarthritis of the thumb.
Methods: Between March 2009 and May 2013, 199 arthroplasties were performed in a single hand center, in 184 patients by 6 hand surgeons. 107 implants were followed up for a mean 137 months (range, 120 to 168 months).

The unique saddle articulation of the trapeziometacarpal joint allows for a wide range of motion necessary for routine function of the thumb. Inherently unstable characteristics of the joint can lead painful instability. The aim of this study was to evaluate the fragment size in relation to the UCL footprint size in patients with an avulsion fracture injury and to find risk factors associated with surgical treatment.
Methods: In a cohort of avulsion fracture injury patients, the largest side of the fragment was divided by the average reported UCL footprint size (ff-ratio), and a logistic regression was performed to find variables associated with surgery.
