Esophageal and esophagogastric junction cancers, version 1.2015. | LitMetric

AI Article Synopsis

Article Abstract

Esophageal cancer is the sixth most common cause of cancer deaths worldwide. Adenocarcinoma is more common in North America and Western European countries, originating mostly in the lower third of the esophagus, which often involves the esophagogastric junction (EGJ). Recent randomized trials have shown that the addition of preoperative chemoradiation or perioperative chemotherapy to surgery significantly improves survival in patients with resectable cancer. Targeted therapies with trastuzumab and ramucirumab have produced encouraging results in the treatment of advanced or metastatic EGJ adenocarcinomas. Multidisciplinary team management is essential for patients with esophageal and EGJ cancers. This portion of the NCCN Guidelines for Esophageal and EGJ Cancers discusses management of locally advanced adenocarcinoma of the esophagus and EGJ.

Objective: To develop and validate a model for preoperative prediction of positive proximal margins for adenocarcinoma of the esophagogastric junction (AEG) by transabdominal approach, and to analyze the safe margin distances for patients with different risks of positive proximal margins.

Materials And Methods: A retrospective analysis was performed on 284 AEG patients who underwent surgery via the transabdominal approach at Hengshui People's Hospital between January 2017 and December 2023. Patients were divided into a training set (=201, first five years) and a test set (=83, last two years).

Background: The aim of this prospective, controlled study was to assess the 5-year follow-up of Laparoscopic Heller-Dor (LHD) in patients with esophago-gastric junction outflow obstruction (EGJOO), compared with achalasia patients (ACH). The management of EGJOO reflects the experience gained with esophageal achalasia, for which LHD has been proven to be an effective long-term treatment. Prospective long-term results of LHD in EGJOO patients are still lacking.

Exploring the relaxation effects of Coptis chinensis and berberine on the lower esophageal sphincter: potential strategies for LES motility disorders.

BMC Complement Med Ther

Department of Pediatrics, E-Da Hospital, I-Shou University, No. 1, Yi-Da Road, Yan-Chao District, Kaohsiung City, 82445, Taiwan, R.O.C..

Article Synopsis
  • Esophageal achalasia is a condition that affects the lower esophageal sphincter (LES), leading to symptoms like difficulty swallowing, chest pain, and weight loss, with traditional treatments providing limited relief.
  • This study investigates the effectiveness of Traditional Chinese Medicine, specifically Coptis chinensis and its active component berberine, in promoting LES relaxation as a potential alternative treatment.
  • Results indicate that both C. chinensis extract and berberine significantly relax the LES by activating potassium channels, suggesting further research is warranted for their therapeutic use in esophageal disorders like achalasia.

Blunt esophageal injury is an exceptionally rare condition, with complete esophageal avulsion being almost unprecedented in adults. This case study details the clinical presentation, surgical management, and postoperative course of a 50-year-old male who sustained a complete esophageal avulsion following blunt abdominal trauma. The patient presented with increasing abdominal pain two hours after falling while stepping up onto a high truck step, striking his upper abdomen on the step.

The Esophageal Adenocarcinoma Study Group Europe (EACSGE) recently proposed a granular histologic classification of esophageal-esophago-gastric junctional adenocarcinomas (EA-EGJAs) based on the study of naïve surgically resected specimens that, when combined with the pTNM stage, is an efficient indicator of prognosis, molecular events, and response to treatment. In this study, we compared histologic classes of endoscopic biopsies taken before surgical resection with those of the surgical specimen, to evaluate the potential of the EACSGE classification at the initial diagnostic workup. A total of 106 EA-EGJA cases with available endoscopic biopsies and matched surgical resection specimens were retrieved from five Italian institutions.
