Descemetorhexis for Fuchs' dystrophy.

Objective: To describe 2 cases of spontaneous corneal clearing after Descemetorhexis: 1 after iatrogenic trauma (Case 1) and 1 as an intentional surgical intervention for Fuchs endothelial dystrophy (Case 2).

Methods: Retrospective case reports.

Results: Full corneal clarity was observed to restore at approximately 1 month after surgery in both cases. Central endothelial cell counts were recorded as 753 and 731 cells/mm(2) in cases 1 and 2, respectively, at last follow-up. Best spectacle corrected visual acuity (BSCVA) at was 6/6 in both cases at 6 weeks and is retained at 9 months.

Conclusions: Selective Descemetorhexis may offer visual rehabilitation without the need for a graft in select cases of Fuchs endothelial dystrophy. Descemetopexy in anticipation of corneal clearing is a viable initial strategy in cases of iatrogenic Descemet trauma with detachment.

Fibromyalgia syndrome (FMS) presents a complex and challenging disorder in both the diagnosis and treatment, with emerging evidence suggesting a role of small fibre pathology (SFP) in its pathophysiology. The significance of the role of SFP in FMS remains unclear; however, recent evidence suggests degeneration and dysfunction of the peripheral nervous system, particularly small unmyelinated fibres, which may influence pathophysiology and underlying phenotype. Both skin biopsy and corneal confocal microscopy (CCM) have consistently demonstrated that ~ 50% of people with FMS have SFP.

Visually significant ocular decompression retinopathy following laser peripheral iridotomy in a patient with primary acute angle-closure glaucoma.

Purpose: We report a single case of ocular decompression retinopathy (ODR) following neodymium-doped yttrium aluminum garnet laser peripheral iridotomy (Nd:YAG LPI) for primary acute angle-closure glaucoma associated with delayed visual recovery secondary to optic nerve head edema and macular thickening.

Observations: A 56-year-old female patient presented to the emergency department with primary acute angle-closure glaucoma. After topical and IV therapy did not improve intraocular pressure (IOP), an Nd:YAG LPI was performed.

Purpose: We demonstrate a novel approach for the definitive treatment of Lisch epithelial corneal dystrophy via an unintentionally staged alcohol keratectomy and intentionally targeted minor limbal excision with cautery.

Methods: A 46-year-old woman presented with visually significant corneal changes, suspected to be Lisch epithelial corneal dystrophy after clinical examination, anterior segment optical coherence tomography, and confocal microscopy. Alcohol keratectomy was performed with complete resolution, but there was visually significant recurrence at 2 years.
