In this paper, we present a new active contours model to segment human leg bones in computed tomography images that is based on a variable-weighted combination of local and global intensity. This model can split an object surrounded by both weak and strong boundaries, and also distinguish very adjacent objects with those boundaries. The ability of this model is required for segmentation in medical images, e.g., human leg bones, which are usually composed of highly inhomogeneous objects and where the distances among organs are very close. We developed an evolution equation of a level set function whose zero level set represents a contour. An initial contour is automatically obtained by applying a histogram based multiphase segmentation method. We experimented with computed tomography images from three patients, and demonstrate the efficiency of the proposed method in experimental results.

Introduction: There is a lack of clinical evidence supporting the decision-making process between high tibial osteotomy (HTO) and unicomparmental knee arthroplasty (UKA) in gray zone indication, such as moderate medial osteoarthritis with moderate varus alignment. This study compared the outcomes between HTO and UKA in such cases and assessed the risk factor for not maintaining clinical improvements.
Materials And Methods: We retrospectively reviewed 65 opening-wedge HTOs and 55 UKAs with moderate medial osteoarthritis (Kellgren-Lawrence grade ≥ 3 and Ahlback grade < 3) and moderate varus alignment (5°< Hip-Knee-Ankle angle < 10°) over 3 years follow-up.

In adulthood, this may result in overload and degenerative changes in the lumbar spine, hip, and knee joints of the longer limb, and the fixed equinus position of the foot of the shorter limb. Patient-specific templating (PST), which is a sister procedure to patient-specific instrumentation (PSI) but hospital-based, is relatively less complex and less expensive than robotics and navigation. However, there are some concerns about the PST including the process of preoperative planning, 3D printing and material, positioning of PST intraoperatively, availability, and clinical value. The purpose of this study was to validate the technical accuracy and reliability of the PST technique in the lab and to report the outcomes of clinical application.

This study aimed to develop a novel reconstruction method for segmental mandibulectomy. In the authors' opinion, reconstruction of the anterior border of the mandibular ramus using a double-arm vascularized fibular flap is important to prevent deformity due to buccal depression and the accumulation of food debris, thereby eliminating masticatory dead space that cannot be filled with prostheses such as implants or dentures. Using conventional reconstruction plates, the reconstructed bone positioned at the anterior border of the mandibular ramus required either fixing with only 1 screw or using 2 plates for stable fixation, making it difficult to position the plates stably.
