Mesalazine-induced pericarditis is rare, and most cases occur at the time of treatment initiation. A 30-year-old man with ulcerative colitis who had experienced remission for 2 years with mesalazine 2,000 mg/day subsequently experienced relapse. Therefore, the mesalazine dose was increased to 4,000 mg.

Background: Mesalazine is an established first-line therapy for inflammatory bowel disease (IBD) and remains the mainstay of treatment for mild to moderate ulcerative colitis (UC). Both mesalazine and UC are rare but recognized causes of myopericarditis. Cardiac magnetic resonance (CMR) is a non-invasive method of assessing for myopericarditis.

Acute interstitial nephritis (AIN) corresponds to a decline in kidney function due to an injury induced by drugs (in the majority of cases), infections, and autoimmune disorders. It is characterized by the presence of an interstitial inflammatory infiltrate in the kidney. Here, we describe a case of a man with a previous medical history relevant to ulcerative colitis (UC) who was admitted due to acute kidney injury (AKI) in the setting of AIN secondary to mesalazine.
