Acute lymphoblastic leukemia (ALL) is a hematologic cancer that mostly affects children. It accounts for over a quarter of ALL pediatric cancers, causing most of the cancer death among children. Previously, we demonstrated that D,L-methadone causes ALL cell apoptosis via μ-opioid receptor 1 (OPRM1)-triggered ER Ca release and decrease in Ca efflux, elevating [Ca].

Methadone is used as an agent for chronic pain and management of opioid use disorder. While similar pharmacologically to other opioids, methadone does have unique characteristics, including long half-life, low cost, and high oral bioavailability. While advantageous in some ways, methadone is associated with unique adverse effects not seen with other opioids (ie, hypoglycemia).

Article Synopsis
  • - Opioid-induced constipation is a major issue, affecting 40% to 60% of patients on opioids without cancer, leading to severe complications.
  • - A 71-year-old man with a history of opioid use disorder, treated with methadone, developed severe constipation and fecal impaction that caused ureter compression and right hydronephrosis.
  • - His condition improved significantly through an aggressive bowel regimen involving stool softeners, laxatives, and enemas, highlighting the risks associated with methadone treatment.

Takotsubo cardiomyopathy (TC) is a syndrome characterized by acute and transient regional systolic myocardial dysfunction. TC often mimics myocardial infarction without obstructive coronary disease. We present a case of a 48-year-old woman who developed TC following the onset of polymorphic ventricular tachycardia in the setting of methadone intoxication.
