Spontaneous bilateral subhyaloid hemorrhage in congenital hypofibrinogenemia.

Retin Cases Brief Rep

From the Department of Retina and Vitreous, L. V. Prasad Eye Institute, GMR Varalakshmi Campus, Vishakapatnam, Andhra Pradesh, India.

Published: November 2014

Purpose: The purpose of this study was to report a case of spontaneous bilateral subhyaloid hemorrhage in congenital hypofibrinogenemia.

Methods: This is a single case report. A 14-year-old girl presented with a spontaneous bilateral decrease in vision. Her history was unremarkable. On examination, visual acuity was 20/80 in the right eye and 20/1200 in the left eye. Fundus examination showed bilateral subhyaloid hemorrhage. Clotting profile showed normal activated partial thromboplastin time and normal prothrombin time with fibrinogen decreased to 50 mg/dL (normal, 200-400 mg/dL).

Results: Therapeutic cryoprecipitate infusion was administered. At 3 months, visual acuity improved to 20/20 with resolution of subhyaloid hemorrhage in both eyes.

Conclusion: Estimation of fibrinogen in the absence of other evaluated causes of blood dyscrasias would help with prompt diagnosis and successful management.

Multifaceted approach to evacuating multi-level premacular hemorrhage in a case of suspected Valsalva retinopathy - a case report.

BMC Ophthalmol

January 2025

Department of Retina and Vitreous, Narayana Nethralaya, #121/C, 1st R Block, Chord Road, Rajaji Nagar, Bengaluru, Karnataka, 560010, India.

Background: Accurate localization of premacular hemorrhages (PMHs) is crucial as treatment strategies vary significantly based on whether the hemorrhage resides within the vitreous gel, subhyaloid space, or beneath the internal limiting membrane (ILM). This report outlines the clinical features, diagnostic findings, and treatment outcomes in a patient diagnosed with a PMH secondary to suspected Valsalva retinopathy.

Methods: This is a retrospective interventional case report.

Diabetic vitrectomy is a highly intricate surgical procedure performed during the advanced stages of diabetic retinopathy (DR). It is used to treat conditions such as tractional or combined retinal detachment, vitreous hemorrhage, and subhyaloid hemorrhage, which are all severe manifestations of proliferative DR. The results of the surgery are uncertain and variable.

Article Synopsis
  • - Terson syndrome (TS) is an eye condition linked to subarachnoid hemorrhage (SAH) that causes bleeding in the eye, leading to visual impairment, and can affect one or both eyes unevenly.
  • - A 52-year-old woman experienced TS after a right middle cerebral artery aneurysm rupture during sex, but surprisingly, she reported significant visual issues in her left eye while the right eye showed more damage.
  • - TS usually causes symptoms in both eyes, but in this case, the symptoms were predominantly found in the eye opposite to the affected hemisphere, making it an unusual presentation.

Article Synopsis
  • Terson syndrome is marked by intraocular hemorrhage related to brain injuries, with a notable incidence of 10-40% following sub-arachnoid hemorrhage.
  • A case study of a 48-year-old woman with sub-arachnoid hemorrhage showed significant visual impairment due to vitreous hemorrhage, which improved after treating an aneurysm in a major artery.
  • Timely ophthalmic evaluations are crucial for diagnosing Terson syndrome in patients with sub-arachnoid hemorrhage, as early intervention can help prevent permanent visual loss.
