Metastatic breast carcinoma can mimic benign cutaneous lesions. Breast surgeons should be aware of skin manifestations to be able to distinguish them and set a proper therapeutic strategy. A clinical case of cutaneous lesion after breast cancer is presented. A 41-year-old woman with a history of left breast cancer underwent a prophylactic right nipple-sparing mastectomy with immediate breast implant reconstruction. After surgery, she attended our service due to a right periareolar rash resistant to medical treatment, accompanied by cutaneous induration and fixed axillary adenopathy. A differential diagnosis of skin metastases was considered. Cutaneous metastases should be the first diagnosis of skin lesions in oncological patients due to the implications in terms of treatment and prognosis. However, differential diagnoses have to be discussed.

Erlotinib, an epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI), is currently used in the therapy of several solid malignancies. This agent has been associated with several dermatological side-effects, the most common being papulo-pustular acneiform rash. Herein we describe a unique skin effect in a patient treated with erlotinib for non-small cell lung cancer.

Phototoxic reaction to oral terbinafine due to Tinea capitis in a child.

Acta Dermatovenerol Croat

November 2024

Prof. Ana Bakija-Konsuo, MD, PhD, Clinic for Dermatovenerology CUTIS, Vukovarska 22, Dubrovnik, Croatia;

We report the case of an 18-month-old boy who developed a phototoxic skin reaction to terbinafine on his scalp, ears, and face in the form of disseminated erythematous plaques, which resembled subacute lupus erythematosus (SCLE) in their clinical presentation. Skin changes appeared a short time after the boy was exposed to sunlight during the period of time when he was treated with oral terbinafine due to Microsporum canis fungal scalp infection. Tinea capitis is a common dermatophyte infection primarily affecting prepubertal children (1).

Hidradenitis suppurativa is a chronic inflammatory disease of the skin with a suppurative-cicatricial outcome affecting the infundibular component of the pilo-sebaceous unit. The lesions are typically localized in the intertriginous and apocrine gland-rich areas. Hidradenitis suppurativa mainly affects patients at a young age and is very often refractory to conventional medical treatment.

Intravascular large B-cell lymphoma (IVLBCL) is characterized by clinical presentations described as B symptoms, consisting of fever, night sweats, and weight loss. Intravascular lymphomas are more frequently diagnosed in elderly patients and are challenging to diagnose because of their nonspecific clinical presentation. Malignant lymphomas are recognized as the leading cause of fever of unknown origin.

Trastuzumab is now increasingly being used as a potent HER2 inhibitor in treating breast cancer, while acneiform rashes sometimes arise as skin-related side effects in patients undergoing treatment with HER2 inhibitors, and their specificity as drug-induced eruptions makes their management quite challenging. Pulsed dye laser (PDL) therapy has been utilized to treat a wide variety of vascular lesions, achieving excellent outcomes. A 595 nm PDL was used to treat a 38-year-old woman suffering from an acneiform rash induced by trastuzumab as neoadjuvant therapy for breast cancer.
