Implantable loop recorders (ILRs) have become a key tool in managing patients with unexplained syncope, and guidelines advise their use in individuals with recurrent, unexplained syncope or palpitations. This case report presents a 16-year-old basketball player, who developed deep venous thrombosis (DVT) following surgical intervention for a displaced tibial tuberosity fracture and forearm fractures. Despite few identifiable thrombotic risk factors, the patient's postoperative course was complicated by unexplained leg pain, fever, and ultimately confirmed DVT. Prompt management with therapeutic anticoagulation and multidisciplinary care led to favourable outcomes.
