Purpose: To compare the surgical outcomes after transumbilical laparoscopic-assisted appendectomy (TULAA) and open appendectomy (OA) at a single institution.

Methods: We compared the surgical outcomes for 94 consecutive patients who underwent TULAA between April 2010 and March 2014 to those for 91 consecutive patients who underwent OA between April 2006 and March 2010.

Results: There were no significant differences in the clinicopathological backgrounds between the two groups. Although the lengths of the operations were similar in both groups, the postoperative hospital stay was significantly shorter in the TULAA group (4.7 days vs. 5.4 days, P = 0.02). The need for abdominal drain insertion was significantly reduced in the TULAA group owing to sufficient intraperitoneal exploration (P = 0.03). The incidence of postoperative complications was also lower in the TULAA group, but the difference was not significant (8.6 % vs. 12.1 %, P = 0.31). In complicated cases, a lower incidence of surgical site infection was confirmed in the TULAA group (6.7 % vs. 20.7 %, P = 0.12).

Conclusion: Our results demonstrated that TULAA provided better surgical outcomes, especially a faster recovery. TULAA could be an effective procedure incorporating both open and laparoscopic techniques, and can be implemented as a standard procedure for the treatment of appendicitis, regardless of disease severity.

Comparison of Transumbilical Laparoscopy-Assisted Appendectomy with Conventional Three-Port Laparoscopic Appendectomy Performed by Pediatric Surgeons in Training for Appendicitis in Children.

J Laparoendosc Adv Surg Tech A

April 2024

Department of Pediatric Surgery, Research Field in Medical and Health Sciences, Medical and Dental Area, Research and Education Assembly, Kagoshima University, Kagoshima, Japan.

Article Synopsis
  • Transumbilical laparoscopy-assisted appendectomy (TULAA) is preferred for treating appendicitis but is harder for trainees to learn than conventional three-port laparoscopic appendectomy (CTPLA) in children.
  • A study analyzed surgical outcomes of 225 pediatric patients with appendicitis treated via either TULAA or CTPLA by pediatric surgeons in training from April 2016 to December 2022.
  • Results showed TULAA had shorter operative and pneumoperitoneum times compared to CTPLA; however, TULAA had a higher—but not statistically significant—rate of surgical site infections, particularly notable in uncomplicated cases.

Article Synopsis
  • The study investigates the effectiveness of transumbilical laparoscopic-assisted appendectomy (TULAA) in treating both uncomplicated (UA) and complicated appendicitis (CA) among children over a 5-year period.
  • Results show a low conversion rate of 3%, with longer operative time and hospital stays for CA cases compared to UA cases, but similar rates of surgical site infections.
  • The findings suggest that TULAA is safe, feasible, and cost-effective for managing both types of appendicitis, showing no significant disadvantage compared to conventional laparoscopic surgery.

Laparoscopic Appendectomy (LPSA) is the first choice for appendectomy in pediatric surgery. Trans-Umbilical Laparoscopic Assisted Appendicectomy (TULAA) is another used technique. We compared both these procedures used for the treatment of acute appendicitis.

Single-center comparison of outcomes between laparoscopic appendectomy and transumbilical laparoscopic assisted appendectomy.

J Pediatr Surg

May 2023

Department of General Surgery, Children's Hospital of Orange County, Orange, CA, USA; Department of Surgery, University of California, Irvine, Orange, California, USA.

Purpose: Appendectomy is the most common pediatric emergency surgery performed to date. This study compared outcomes between laparoscopic appendectomy (LA) and transumbilical laparoscopic assisted appendectomy (TULAA) for 1154 uncomplicated patients across 5 years at a single institution. Primary outcomes include length of stay (LOS), post-operative complications, pain score, and operating room (OR) time.
