Unlabelled: Transcatheter aortic valve implantation (TAVI) procedure is a well-established therapeutic measure for severe aortic stenosis with expanding indications. We present a case of a patient who had undergone a TAVI procedure during cardiopulmonary resuscitation on the grounds of cardiogenic shock.
Learning Objectives: During the past decade, as platform and delivery technology regarding the implementation of transcatheter aortic valve implantation (TAVI) have improved, procedure indications expanded both to low and very high-risk patients.

Background Patients undergoing transcatheter aortic valve implantation (TAVI) are often elderly, and perioperative and long-term risk assessments should primarily consider cognitive function, comorbidities, and procedural complexity. This study investigated the association between cognitive function and mortality in patients with severe aortic valve stenosis (AS) who underwent TAVI. Methodology This single-center, retrospective cohort study consecutively registered patients who underwent TAVI between December 2014 and December 2018.

Aortic valve replacements, both surgical and transcatheter, are nowadays widely employed treatments. Although clinically effective, these procedures are correlated with potentially severe clinical complications which can be associated with the non-physiological haemodynamics that they establish. In this work, the fluid dynamics changes produced by surgical and transcatheter aortic valve replacements are analysed and compared with an ideal healthy native valve configuration, employing advanced fluid-structure interaction (FSI) simulations.

Background: The combination of symptomatic aortic stenosis (AS) and subaortic membrane (SAM) is rare, and the haemodynamic consequences of this combination are challenging to diagnose and manage.
Case Summary: We present the case of a 78-year-old male who presented with symptomatic AS and chronic heart failure. On echocardiography, the combination of AS and SAM was diagnosed.

Transcatheter aortic valve implantation (TAVI) is a safe and effective procedure for the treatment of aortic stenosis. With the recently broadened indications, there is a larger cohort of patients likely to outlive their first transcatheter heart valve (THV). This review discusses relevant lifetime planning considerations, focusing on the utility of preprocedural computed tomography imaging to help implanters future-proof their patients who are likely to outlive their first valve.
