Sever's disease is juvenile osteochondrosis of calcaneus, manifested with posterior heel pain and mild swelling, usually in young athletes, caused by repetitive microtrauma. The standard diagnostic procedures include radiographs and ultrasound examination. It is a self-limiting condition, usually treated conservatively. We present a case of Sever's disease complicated with calcaneal osteomyelitis. A 9-year-old boy with a several month history of left heel pain was admitted to our institution for further clinical evaluation and therapy. Bilateral Sever's disease was clinically diagnosed, confirmed by radiographs and ultrasound examination. Magnetic resonance imaging (MRI) showed osteolytic zone in the posterior left calcaneus with peripheral enhancement after contrast administration and surrounding bone and subcutaneous edema, suggestive of osteomyelitis. After six weeks of parenteral antibiotic treatment, the patient's condition improved. Follow up MRI performed 3 months later showed significant regression of osteolytic lesion and replacement with granulation tissue surrounded with new bone formation. To our knowledge and literature search, this is a previously unreported complication of Sever's disease. MRI should be done in all atypical cases of Sever's disease to rule out any other possible disorders including osteomyelitis.

Article Synopsis
  • Sever's disease is a common cause of foot pain in children and may be linked to a higher prevalence of ADHD, particularly among boys.
  • A study involving 88 children with Sever's disease found 70.5% had ADHD compared to just 13.3% in a control group.
  • The findings suggest that children with Sever's disease, especially males, should be assessed for ADHD, as this could provide important insights for treatment.

Article Synopsis
  • - The use of recombinant human growth hormone (GHT) in children has grown significantly, with applications expanding from just treating growth hormone deficiencies to a wider range of conditions, increasing threefold over the past 20 years.
  • - Despite its benefits, there are rising concerns about safety, especially orthopedic issues like carpal tunnel syndrome and various growth-related diseases, attributed to factors such as hormonal changes and rapid growth associated with GHT.
  • - The review emphasizes the need for careful monitoring and coordinated care among pediatricians, endocrinologists, and orthopedic surgeons to minimize risks, and suggests future research to better understand injury patterns and improve clinical guidelines for managing orthopedic injuries in children on GHT.

Article Synopsis
  • This prospective cohort study investigated factors that lead to Sever's disease in junior gymnasts, analyzing 74 limbs from 37 athletes over six months.
  • The evaluation included demographic data, flexibility, foot alignment, trunk function, and balance tests to assess the incidence of the disease.
  • Findings indicated that gymnasts with Sever's disease had lower arch height ratios and balance scores, suggesting that preventive measures like arch supports and balance training could be beneficial.

Article Synopsis
  • Lower limb apophyseal injuries are common in children and adolescents, potentially impacting their quality of life and leading to reduced sports participation during important developmental years.
  • The research developed self-administered screening tools for two prevalent conditions: Sever's disease (calcaneal apophysitis) and Osgood-Schlatter's disease (tibial tuberosity apophysitis), using expert consensus and literature support.
  • The tools demonstrated high diagnostic accuracy, with potential to assist clinicians in preclinical screening and facilitate larger epidemiological studies to better understand the prevalence of these injuries.
