Purpose: To demonstrate the Modified Balloon Nose Cone Technique to avoid passage of the aortic valve in endovascular branched arch repair.
Technique: The technique is demonstrated in a 54-year-old patient after previous open repair of the ascending aorta and mechanical aortic valve replacement due to type A aortic dissection. The delivery system of a custom-made stent-graft with 3 inner branches was modified by subtotally sawing off its nose cone.

Background: Lower extremity artery disease is increasingly prevalent, and complex lesions such as calcified chronic total occlusions pose significant challenges during endovascular therapy. The needle rendezvous technique, which involves puncturing a needle toward the guidewire within the lesion or lumen and advancing the guidewire into the needle lumen to achieve guidewire externalization, offers a potential solution. If device passage remains challenging, the Rendezvous-PIERCE technique can be subsequently employed by advancing the needle over the externalized guidewire to modify the lesion directly.

Background: Severe calcification often prevents device passage and balloon expansion in cases of lower extremity artery disease. To address this limitation, we introduced a novel calcium modification technique called Rendezvous-PIERCE (R-PIERCE).
Methods: A needle was inserted in a retrograde manner and advanced to touch the tip of an antegrade guidewire within the lesion.
