Background: Lung cancer cells have been reported to produce cytokines, resulting in systemic reactions. There have been few reports showing that these cytokines induced the formation of an inflammatory mass around lung cancers.

Case Presentation: We encountered a patient with a pleomorphic carcinoma of the lung. This tumor produced interleukin (IL)-8, granulocyte colony-stimulating factor and IL-6, which in turn recruited inflammatory cells, such as CD8 positive lymphocytes, around the tumor, resulting in a rapidly growing tumor shadow.

Conclusion: 18 F-fluoro-deoxy-glucose positron emission tomography, in addition to a conventional radiological approach such as computed tomography, may detect immunological responses around a tumor.

Pleomorphic carcinoma of the lung is highly malignant and occurs mostly in the upper lobes of smokers; however, no reports exist of new occurrences from the resection margins of the previous ground-glass opacities. In our case, a 73-year-old man underwent an initial partial resection of the left upper lung for ground-glass opacities, with progressive thickening of the resection margin during the following 10 months. Radical resection was performed, resulting in the diagnosis of a newly arising pleomorphic carcinoma from previously resected margins.

A male in his seventies presented with lung cancer in the right lower lobe. The surgically resected specimen revealed a pleomorphic carcinoma featuring an adenocarcinoma component with lepidic, acinar, and papillary patterns, alongside a spindle cell component spreading along the pulmonary artery wall, resembling intimal sarcoma.
