Living kidney donors were analyzed in different respects in our long-term study. There were important findings, which include the following: 1. Kidney donors live longer, most likely due to a selection of healthy individuals. 2. Remnant kidney function increases for a long period, both in younger and older donors. There is a subsequent decrease in function that is more apparent in the elderly. 3. Hypertension is a common long-term finding, suggesting the need for regular blood pressure check-ups after donation. 4. Altruistic reasons were the most common motives for donation. 5. Spouses seem to be the best choice, since quality of life improves for the entire family. 6. For the donor, the transplantation results are the most important. Not only immunosuppressive therapy, but also treating co-morbidity in the recipients is therefore important. 7. Personal experience supports the use of living donors.

Vascularized composite allograft (VCA) transplantation represents a significant advancement in reconstructive surgery and offers hope to individuals who experienced congenital disorders or severe tissue injuries to restore physical appearance, function, and enhance quality of life. VCA recovery introduces complexities to conventional solid organ recovery, and there remain concerns regarding the potential impact of VCA recovery on non-VCA organs for transplant. The current retrospective study examines deceased donor characteristics and observed-to-expected (O/E) organ yield ratios for 51 VCA donors recovered in the US between July 4, 2014 and March 31, 2024, compared with a contemporary cohort of non-VCA donors recovered in 2023.

Background: This study assesses the impact of human leukocyte antigen (HLA)-DR mismatch and donor-estimated glomerular filtration rate (eGFR) on outcomes of living donor kidney transplantation (LDKT), which are especially relevant to the availability of multiple donors and paired kidney exchanges.

Methods: Using data from the Scientific Registry of Transplant Recipients (SRTR), we retrospectively analyzed graft survival in adult LDKT recipients transplanted between January 2013 and September 2022. Recipients with 0 HLA-DR mismatches were compared to those with 1-2 HLA-DR mismatches.
