Background: GlideScope (GS) is used in pediatric endotracheal intubation (ETI) but requires a different technique compared to direct laryngoscopy (DL).

Purposes: This article was written to evaluate the efficacy of exploration-based learning on procedural performance using GS for ETI of simulated pediatric airways and establish baseline success rates and procedural duration using DL in airway trainers among pediatric providers at various levels.

Methods: Fifty-five pediatric residents, fellows, and faculty from Pediatric Critical Care, NICU, and Pediatric Emergency Medicine were enrolled. Nine physicians from Pediatric Anesthesia benchmarked expert performance. Participants completed a demographic survey and viewed a video by the GS manufacturer. Subjects spent 15 minutes exploring GS equipment and practicing the intubation procedure. Participants then intubated neonatal, infant, child, and adult airway simulators, using GS and DL, in random order. Time to ETI was recorded.

Results: Procedural performance after exploration-based learning, measured as time to successful ETI, was shorter for DL than for GS for neonatal and child airways at the.05 significance level. Time to ETI in adult airway using DL was correlated with experience level (p =.01). Failure rates were not different among subgroups.

Conclusions: A brief video and period of exploration-based learning is insufficient for implementing a new technology. Pediatricians at various levels of training intubated simulated airways faster using DL than GS.

The process of extracting oil and gas via borehole drilling is largely dependent on subsurface structures, and thus, well log analysis is a major concern for economic feasibility. Well logs are essential for understanding the geology below the earth's surface, which allows for the estimation of the available hydrocarbon resources. The incompleteness of these logs, on the other hand, is a major hindrance to downstream analysis success.

Humans adapt their locomotion seamlessly in response to changes in the body or the environment. It is unclear how such adaptation improves performance measures like energy consumption or symmetry while avoiding falling. Here, we model locomotor adaptation as interactions between a stabilizing controller that reacts quickly to perturbations and a reinforcement learner that gradually improves the controller's performance through local exploration and memory.

Near-Infrared Autofluorescence Signatures of Single- vs Multigland Disease in Primary Hyperparathyroidism.

JAMA Otolaryngol Head Neck Surg

November 2024

Department of Endocrine Surgery, Endocrinology and Metabolism Institute, Cleveland Clinic, Ohio.

Importance: The success of parathyroidectomy depends on accurate intraoperative localization and identification of all diseased glands in parathyroid exploration based on surgeon expertise to prevent persistent hyperparathyroidism. Near-infrared autofluorescence (NIRAF) imaging has recently emerged as a promising adjunctive intraoperative tool for localizing parathyroid glands; however, its potential utility in the assessment of parathyroid glands has yet to be established.

Objective: To analyze the differences in NIRAF signatures of parathyroid glands in single vs multiple glands in primary hyperparathyroidism (pHPT).

A deep learning-based cascade algorithm for pancreatic tumor segmentation.

Front Oncol

August 2024

College of Computer Science and Technology, Xidian University, Xi'an, Shaanxi, China.

Pancreatic tumors are small in size, diverse in shape, and have low contrast and high texture similarity with surrounding tissue. As a result, the segmentation model is easily confused by complex and changeable background information, leading to inaccurate positioning of small targets and false positives and false negatives. Therefore, we design a cascaded pancreatic tumor segmentation algorithm.

Biped Robots Control in Gusty Environments with Adaptive Exploration Based DDPG.

Biomimetics (Basel)

June 2024

Graduate School of Information, Production and Systems, Waseda University, Kitakyushu 808-0135, Japan.

Article Synopsis
  • Bipedal robots are increasingly used in various environments, but maintaining balance during wind disturbances is a major challenge due to their greater complexity compared to wheeled robots.
  • To address this, researchers have developed an adaptive bio-inspired exploration framework based on the Deep Deterministic Policy Gradient (DDPG) approach, which allows robots to adjust to wind forces and optimize their stability.
  • The incorporation of Hindsight Experience Replay (HER) and a reward-reshaping strategy enhances the training process, leading to faster adaptations and improvements in walking efficiency under difficult conditions.
