Although Iselin's disease, apophysitis of the fifth metatarsal base, is not infrequent in clinical practice, it is accepted as a rare cause of lateral foot pain in young adolescents. Usually a simple clinical examination and radiographs are sufficient for diagnosis. We present a patient with Iselin's disease and discuss its clinical and radiographic characteristics, differential diagnosis, aetiopathogenesis and treatment.

Various etiologies and risk factors contribute to foot pain in children and adolescents, including conditions such as Kohler's disease, Sever's disease, Iselin's disease, rigid flat foot, accessory navicular, Freiberg's disease, sesamoiditis, os trigonum syndrome, and more. High-frequency musculoskeletal ultrasonography can show both the bone surface and the surrounding soft tissue clearly from various angles in real-time, thereby providing a higher level of detail that is helpful for identifying the etiology of foot pain and monitoring disease progression compared with other imaging modalities. This review provides an overview of the epidemiology, pathophysiology, clinical manifestations and characteristic ultrasonographic findings of select foot pain conditions in children and adolescents.

Article Synopsis
  • Os vesalianum pedis (OVP) is a rare bone in the foot near the 5th metatarsal that is usually asymptomatic but can sometimes cause lateral foot pain and mimic an avulsion fracture.
  • A case involving a 62-year-old man showed that his OVP was initially misdiagnosed as an avulsion fracture after he suffered an inversion injury.
  • Treatment for OVP is generally conservative, though surgery may be required if non-surgical methods fail, highlighting the importance of correctly diagnosing it to avoid unnecessary procedures.

We report the case of a 55-year-old female with a rare presentation and different pathophysiology of Iselin's disease. Iselin's disease is a rare condition caused by traction apophysitis of the peroneus brevis tendon at the base of the fifth metatarsal bone. It is usually a case in the adolescent age group due to their repetitive use of the peroneus tendon in daily activities, but not in the elderly group.

Traction apophysitises are a well-known entity in the orthopedic world that are generally thought of as conditions of the pediatric population secondary to overuse and repetitive microtrauma. Many have been named for their original descriptors, and therefore the purpose of this review is to highlight the backgrounds of these physicians, pay tribute to those who came before, and standardize the definitions of these oft-used eponyms. Iselin's disease, Little League Elbow, Osgood-Schlatter disease, Sever's disease, and Sinding-Larsen-Johansson disease will be discussed in brief, followed by a historical background presenting the original description of the named author as well as a short biography.
