Aim: To compare the clinical effectiveness of the treatment of choledocholithiasis by laparoscopic common bile duct (CBD) exploration and by endoscopic sphincterotomy (EST).

Materials And Methods: A meta-analysis of studies about CBD stones was performed to analyze EST in comparison with laparoscopic CBD exploration procedures. Trials were identified by searching the Medline, EMBASE, PubMed, CBM, and CNKI databases from January 1990 to December 2012 for laparoscopic CBD exploration or EST for CBD stones.

Results: Fifteen studies were identified in the meta-analysis. The incidence of bleeding or pancreatitis in the EST group was higher than that in the laparoscopic group. However, the incidence of bile leakage in the EST group was lower than that in the laparoscopic group. The differences in cases of retained stones or total complications were not statistically significant between the laparoscopic and EST groups (P>.05). There were more successful cases in the laparoscopic group than in the EST group (P<.05). Hospital cost was less in the laparoscopic group than in the EST group (P<.05). Mean operation time and hospital stay in the laparoscopic group were shorter than those in the EST group (P<.05).

Conclusions: To some degree, laparoscopic treatment of the CBD may be a better way of removing stones than EST.

Background: Stone impaction is an obstacle to successful laparoscopic common bile duct exploration (LCBDE). This study aims to identify the incidence, operative difficulties and techniques used to disimpact and remove impacted stones during LCBDE.

Methods: Prospectively collected data from a large series of LCBDE.

Background: Adolescent idiopathic scoliosis (AIS) is a complex three-dimensional deformity, and up to now, there has been no literature reporting the analysis of a large sample of X-ray imaging parameters based on artificial intelligence (AI) for it. This study is based on the accurate and rapid measurement of x-ray coronal imaging parameters in AIS patients by AI, to explore the differences and correlations, and to further investigate the risk factors in different groups, so as to provide a theoretical basis for the diagnosis and surgical treatment of AIS.

Methods: Retrospective analysis of 3192 patients aged 8-18 years who had a full-length orthopantomogram of the spine and were diagnosed with AIS at the First Affiliated Hospital of Zhengzhou University from January 2019 to March 2024.

Anti-seizure medications (ASMs) are specific types of anticonvulsants used to treat epileptic seizures. However, several studies have shown an association between ASMs and an increased risk of hematological disorders, such as thrombocytopenia, aplastic anemia, and platelet function disorders leading to prolonged bleeding times. This review explores the existing literature on this topic, investigating a wide variety of ASMs, ranging from first-generation medications to newer ones.

Article Synopsis
  • Lennox-Gastaut syndrome (LGS) and Dravet syndrome (DS) are severe epilepsy types often seen in young children, and a CBD medication (Epidyolex®) has been reimbursed in the Netherlands since December 2022 to help treat their seizures.
  • A cost-effectiveness analysis using a Markov model showed that using CBD along with usual care for LGS resulted in additional costs of €28,338 but provided increased quality-adjusted life-years (QALYs) of 1.318, making it cost-effective below the €80,000 threshold.
  • For DS patients, CBD plus usual care showed a cost saving of €23,642 while increasing QAL
