Background And Objective: To evaluate the usefulness of a 20-gauge cannula to maintain a self-sealing sclerotomy wound after 23-gauge phacofragmentation.
Patients And Methods: This retrospective study compared the suture rates after 23-gauge phacofragmentation when the 23-gauge cannula was temporarily replaced with a 20-gauge valved metal cannula versus when the 23-gauge fragmatome was inserted at the sclerotomy site without a cannula.
Results: Whereas a sclerotomy was sutured in all 31 eyes in the without-cannula group, only one eye of 14 in the cannula group required a sclerotomy suture (P < .0001). However, there was one case of fragmatome tip fracture during fragmentation in the cannula group.
Conclusion: A self-sealing sclerotomy wound can be maintained without suturing by using a 20-gauge metal cannula, but fragmatome tip fracture can occur during fragmentation.

Background: Fibrin glue is an absorbable blood-derived product, a biological tissue adhesive which imitates the final stages of the coagulation cascade, it produces a firm clot, forming a seal along the whole length of the wound, the resultant fibrin clot degrades physiologically into granulation tissue 2 weeks after the application. Biological glue has been used extensively in many forms of surgical procedures. Its use in eye surgery has grown lately as we have evidence that showed it was effective in securing conjunctival grafts in pterygium surgery, in securing wounds after glaucoma surgery and more importantly in 20G and 23G vitrectomy.

Introduction: We present a variation in the slit-lamp microscope re-bubbling technique for DMEK detachment in which a paracentesis is used as a self-sealing valve.
Methods: Of twenty-eight consecutive DMEK cases, five (18%) required re-bubbling. A 20-gauge side port blade was used to make a paracentesis in the inferior temporal quadrant.

Superficial veins of the upper extremity are the primary location for placement of peripheral IV catheters (PIVC). It is believed that a significant portion of PIVCs placed may cross or abut valves and branching veins or occlude a significant portion of the vein, limiting the ability to aspirate blood from the PIVC. Two separate clinical investigations using ultrasound were performed to understand the potential interaction between PIVCs and the vein lumen and the venous valves and branches of the superficial veins of the upper extremity.
