Cerebral beta-amyloid accumulation is the key initiator of Alzheimer's disease (AD) pathology. Most familial early-onset AD mutations in the APP, PSEN1/2 genes increase the ratio of Abeta42:Abeta40, which drives beta-amyloid accumulation in the brain. In 2001, the late Steve Wagner, Maria Kounnas, and I directed an agnostic high-throughput screen for compounds that would reverse the Abeta42:Abeta40, ratio, and discovered the first non-NSAID (second generation) gamma secretase modulators (GSM) at TorreyPines Therapeutics.

Background: Older adults with cognitive impairments will benefit from multicomponent interventions include cognitive training, exercise, and lifestyle modifications. Many digital therapeutic products predominantly focus on computerized cognitive training, lacking effective approaches to other crucial interventions. This study aimed to investigate the long term effects of multidimensional comprehensive cognitive intervention training program - Brain and Body Rehab Training (BBRT), which integrates multidomain cognitive training with physical-cognitive training and multidimensional lifestyle interventions on cognitive performance in participants with cognitive impairment after a 8-month follow-up.

Background: Occupation complexity during adulthood may contribute to cognitive reserve in later life. Research on occupational complexity and cognitive function has focused on high-income countries, where there is a large proportion of individuals with complex occupations. Thus, it is important to investigate this association in the context of low- and middle-income countries where there is greater variance in occupation complexity and where there is a higher proportion of lower-complexity occupations due to educational and other socioeconomic limitations.

Background: Recognizing perceived stress as a modifiable cognitive risk factor, mindfulness-based programs emerge as promising for stress mitigation in older adults with Mild cognitive impairment (MCI). However, existing research, primarily observational and focused on chronic patients and caregivers, necessitates developing and evaluating MCI-specific mindfulness interventions.

Background: With the changing care landscape for early Alzheimer's disease (AD), optimizing the diagnostic and management process is key. This study assessed the correlation between patient and physician characteristics and outcomes related to the diagnosis, referral, and treatment process for early AD in community-based settings.
Method: This cross-sectional study conducted between August and September 2023 abstracted medical chart data for patients aged 50-89 years who were diagnosed with early AD (mild cognitive impairment [MCI] or mild AD) within the past 2 years and had a clinic visit within the past year.
