Discerning between the concepts of difficulty and usefulness of a molecular ranking classification is of significant importance in virtual design chemistry. Here, both concepts are viewed from the statistical and practical point of view according to the standard definitions of enrichment and statistical significance p-values. These parameters are useful not only to compare distinct rankings obtained for the same molecular database, but also in order to compare the ones established in distinct molecular sets from an objective point of view.

An important impediment to the incorporation of artificial intelligence-based tools into healthcare is their association with so-called black box medicine, a concept arising due to their complexity and the difficulties in understanding how they reach a decision. This situation may compromise the clinician's trust in these tools, should any errors occur, and the inability to explain how decisions are reached may affect their relationship with patients. Explainable AI (XAI) aims to overcome this limitation by facilitating a better understanding of how AI models reach their conclusions for users, thereby enhancing trust in the decisions reached.

Background And Aim: Stent placement for biliary drainage in patients with malignant hilar biliary obstruction (MHBO) has been a topic of long-standing debate, and the best approach remains controversial. Therefore, we aimed to evaluate the efficacy, safety, and removability of multi-hole fully covered self-expandable metal stents (MH-FCSEMSs) in a preclinical experiment using swine hilar bile duct obstruction (HBDO) models and to assess the feasibility and safety of stent placement in patients with MHBO.

Methods: Three minipigs underwent endoscopic retrograde cholangiopancreatography (ERCP)-guided endobiliary-radio frequency ablation (EB-RFA) to establish Bismuth type II hilar bile duct stenosis models.
