: Surgical resection remains an important treatment of choice for a large number of liver tumors in children. Sometimes, if a tumor infiltrates a large part of the liver, after resection, the future liver remnant (FLR) is not enough for normal liver function. The size of the FLR is one of the determining factors for resectability as postoperative liver failure (PLF) is the most severe complication after partial hepatectomy.

This case report presents the rare association of Tessier number 4 and 5 facial clefts with a palatal cleft in a 4-month-old infant, encountered during a humanitarian mission in Sabou, Burkina Faso. The patient, born with bilateral facial clefts and right anophthalmia, was planned for a 2-step surgical approach. The first surgery addressed the facial clefts, including a modified unilateral cleft lip repair and Z-plasty techniques for the left Tessier number 5 cleft.

A 4-month-old male infant (weight 6 kg, height 67 cm) with heart failure was diagnosed with an isolated 8.9 mm right coronary artery fistula draining into the left ventricle (LV), identified via transthoracic echocardiography and computed tomography angiography. The large, tortuous, and aneurysmal fistula was treated using a minimally invasive percoronary approach, avoiding the high risks of surgery and the challenges of percutaneous closure.
