Arcuate line hernia is considered a surgical rarity. This type of hernia is characterized by protrusion of intraperitoneal structures in a concave parietal fold in the abdominal wall. In this report, we aim to describe the diagnostic images of 2 cases of arcuate line hernia. Laparoscopic repair using a polypropylene mesh with a preattached inflatable balloon has been illustrated as well.

Article Synopsis
  • Spigelian hernia is a rare type of abdominal wall hernia that often goes unnoticed until it becomes incarcerated, usually forming where the arcuate line meets the lateral part of the rectus abdominis muscle.
  • A case involved a 44-year-old woman with a painful lump in her abdomen, which was suspected to be an incarcerated atypical hernia; imaging confirmed the presence of a Spigelian hernia, despite initial thoughts of a different condition due to her use of interferon.
  • Diagnosing Spigelian hernias is challenging because symptoms can be nonspecific, and the high risk of incarceration suggests that even less obvious cases should be surgically treated.

Background: Parastomal hernia is a major long-term complication after abdominoperineal resection. Extraperitoneal colostomy has been proposed as an effective step for parastomal hernia prevention, but it has not been widely used as it is technically demanding and time-consuming. We proposed a modified approach for extraperitoneal colostomy creation by entering the extraperitoneal space through the arcuate line of the posterior rectus sheath.

Although it is uncommon, gastroesophageal reflux disease can present after Roux-en-Y gastric bypass, and it is usually related to technical errors. Hiatal hernia, a narrow calibrated gastrojejunostomy and a long gastric pouch are all factors associated with the development of pathologic gastroesophageal reflux. Techniques are available to treat this condition, such as fundoplications with the gastric remnant, gastropexy to the arcuate ligament, teres ligament repair, or sphincter augmentation with the LINX device.
