Intrahepatic splenosis is an uncommon condition that can present a significant diagnostic challenge, often masquerading as more sinister hepatic lesions. We report a perplexing case of a 56-year-old female with a history of splenectomy who presented with liver masses initially suspected to be hepatocellular carcinoma (HCC). Despite advanced imaging techniques, including ultrasonography, computed tomography (CT), and magnetic resonance imaging (MRI), the lesions convincingly mimicked HCC.

Background: Heterotopic splenic tissue can occur following splenectomy and is typically asymptomatic, often discovered incidentally during imaging for other conditions. This benign condition may mimic malignant processes, posing diagnostic challenges especially in patients with a history of cancer or concurrent malignancy.
Case Description: We report the case of a 60-year-old male with a history of well-controlled hypertension and a splenectomy following a traumatic injury at age 7.

A 61-year-old man underwent splenopancreasectomy for a 3-cm neuroendocrine tumor of the body of the pancreas (G2, pT1 pN0, Ki67 3%). Five months after surgery 68 Ga-DOTATOC PET/CT showed increased radiotracer uptake in a solid tissue of the splenic fossa, possibly referring to a splenosis nodule. After 19 months, a further 68 Ga-DOTATOC PET/CT showed a significant functional and dimensional increase of the previously detected tissue and the appearance of a new finding in the left lateral abdominal wall.
