Unlabelled: Myocarditis and pericarditis, or myopericarditis, is a rare, albeit life-threatening, cardiac complication of coronavirus disease 2019 (COVID-19). Although most patients recover from myocardial inflammation within weeks of the acute infection, there are concerns about acute and long-term myocardial injury. Coronary microvascular dysfunction and myocardial inflammation in the affected myocardium might be key factors in developing acute COVID-19-associated myopericarditis.

Objective: Assessing myocardial perfusion in acute myocardial infarction is important for guiding clinicians in choosing appropriate treatment strategies. Echocardiography can be used due to its direct feedback and bedside nature, but it currently faces image quality issues and an inability to differentiate coronary macro- from micro-circulation. We previously developed an imaging scheme using high frame-rate contrast-enhanced ultrasound (HFR CEUS) with higher order singular value decomposition (HOSVD) that provides dynamic perfusion and vascular flow visualization.

Acute, intermittent exposure to mild hypoxia increases sympathetic nervous system activity, decreases arterial shear rate, and increases blood pressure (BP) in young men. Given that the BP-raising effects of intermittent hypoxia (IH) are less evident in young women, we sought to examine the influence of sex on macro- and microvascular function following IH. BP, macrovascular function [brachial artery flow-mediated dilation (FMD)], microvascular function [reactive hyperemia area under the curve (RHAUC)], and brachial artery shear rate [shear rate area under the curve (SRAUC)] were measured before and following 30 min of IH in 18 young adults (8 women, 23 ± 5 yr).

Continuous monitoring of the hemodynamic coherence between macro and microcirculation is difficult at the bedside. We tested the role of photoplethysmography (PPG) to real-time assessment of microcirculation during extreme manipulation of macrohemodynamics induced by the cardiopulmonary bypass (CPB).We analyzed the alternating (AC) and direct (DC) components of the finger PPG in 12 patients undergoing cardiac surgery with CPB at five moments: (1) before-CPB; (2) CPB-start, at the transition from pulsatile to non-pulsatile blood flow; (3) CPB-aortic clamping, at a sudden decrease in pump blood flow and volemia.
