Article Synopsis
  • Laryngocele is a rare condition characterized by an abnormal air-filled dilation in the laryngeal ventricle, mostly occurring unilaterally, with bilateral cases being highly uncommon.
  • The article details a case of a 57-year-old male with respiratory distress from bilateral laryngoceles, confirmed through CT imaging, leading to a tracheostomy and successful surgical excision.
  • A literature review shows that laryngoceles are often asymptomatic and typically diagnosed around age 50, highlighting the need for timely medical intervention and further research on this condition.

We present a rare case of bilateral pharyngocoeles in a patient with symptomatic neck swelling prominently seen with the Valsalva manoeuvre. Pharyngocoeles have only been reported a handful of times in the literature. Due to their rarity, they can easily be misdiagnosed as a laryngocoele, Zenker's diverticulum, or jugular venous phlebectasia.

Phlebectasia is the abnormal saccular fusiform dilatation (without tortuosity) of a vein, which may affect any vein. Its etiology remains unknown and it is seen usually in the pediatric age group as a unilateral lower neck swelling, mostly on the right side. The other differential diagnoses of the neck swellings which increase in size during a Valsalva manoeuver are tumors or cysts of the upper mediastinum, external laryngeal diverticulum or laryngoceles.

Pharyngocele is a rare pathology of the pharynx caused by the laxity of the thyrohyoid membrane. Only about 60 true lateral pharyngocele cases have been reported in the literature over the last 133 years. Laryngocele is a close differential, and the two are difficult to tell apart.
