Objective: To determine the incidence of fibular penetration during placement of the Synthes® locking TPLO plate with and without the use of a jig.

Study Design: Cadaveric, experimental study.

Sample Population: Cadaveric paired pelvic limbs (n = 8) from skeletally mature dogs.

Methods: Limbs were assigned to 1 of 2 groups (jig-less-TPLO = no jig used; jig-TPLO = jig used). Synthes® locking TPLO plates were applied using proximal screws of excessive length to facilitate identification of fibular penetration. Screw tip locations were identified by dissection and frequency of fibular penetration was compared between groups.

Results: None of the jig-TPLO limbs and 6 (75%) of jig-less-TPLO limbs had fibular penetration, a difference that was statistically significant. Fibular penetration was most frequently associated with the most proximal screw.

Conclusions: A significantly higher incidence of fibular penetration occurs when Synthes® locking TPLO is performed without use of a jig.

Fractures of the proximal humerus account for 4%-8% of injuries to the appendicular skeleton. Most are stable, minimally displaced osteoporotic fractures in the elderly, and are the result of low-energy falls. A large majority of these patients regain adequate shoulder function without operative intervention.

Background: Alveolar Rhabdomyosarcoma is a profoundly malignant soft-tissue sarcoma that predominantly affects children and adolescents. However, the medical field lacks consensus regarding the optimal surgical approach to be undertaken in cases where this tumor causes local bone destruction in the upper limb.

Case Description: A 17-year-old male presented a mass in his left forearm and CT and MRI indicated that the mass had penetrated the ulnar cortex and infiltrating the medulla, resulting in the formation of an eccentric trans-ventricular tumor focus.

Unveiling Syndesmotic Malreduction: A Proof-of-Concept towards Portable Ultrasound Detection.

Arch Bone Jt Surg

January 2024

Foot & Ankle Research and Innovation Lab (FARIL), Department of Orthopaedic Surgery, Massachusetts General Hospital, Harvard Medical School, Boston, MA, USA.

Article Synopsis
  • The study aimed to assess how effective portable handheld ultrasound (P-US) is for diagnosing fibular rotation issues at the distal tibiofibular joint after an injury called syndesmotic disruption.
  • Researchers used four cadaver specimens to simulate different malreduction conditions after certain ligaments were cut, then used ultrasound to evaluate the gaps between the tibia and fibula.
  • Results indicated that the posterior gap penetrance was quite accurate in identifying rotational malreduction, with high specificity and positive predictive value, while the anterior gap penetrance had limited effectiveness.

Local Antibiotic Therapy for Open Long Bone Fractures: Appropriate Prophylaxis or Unnecessary Exposure for the Orthopedic Trauma Patient?

Department of Surgery, Division of Trauma, Critical Care, Burns, and Emergency Surgery, College of Medicine, University of Arizona, Tucson, Arizona 85724, USA.

Introduction: Prophylactic local antibiotic therapy (LAbT) to prevent infection in open long bone fracture (OLBF) patients has been in use for many decades despite lack of definitive evidence confirming a beneficial effect. We aimed to evaluate the effect of LAbT on outcomes of OLBF patients on a nationwide scale.

Materials And Methods: In this retrospective analysis of 2017-2018 American College of Surgeons-Trauma Quality Improvement Program database, all adult (≥18 years) patients with isolated OLBF (non-extremity-Abbreviated Injury Scale < 3) were included.
