In this Special Section, the relationships among attention deficit hyperactivity disorder (ADHD), impulsivity, and alcohol abuse are explored by 4 diverse studies. Among these excellent studies, the constructs indicated in the issue title varied in definitions, measurement methods, and interpretive implications. The experimental approaches also varied, including cross-sectional examination of candidate genes in children, determination of long-term outcomes, laboratory tasks to measure attention and inhibition, and alcohol administration. These diverse topics and approaches yielded insights into the etiology and effects of alcohol abuse and indicated research directions that will advance this field.

Reinforcement learning studies propose that decision-making is guided by a tradeoff between computationally cheaper model-free (habitual) control and costly model-based (goal-directed) control. Greater model-based control is typically used under highly rewarding conditions to minimize risk and maximize gain. Background: Wernicke-Korsakoff encephalopathy is a metabolic disease caused by vitamin B1 deficiency that predominantly affects alcoholic patients. Its clinical picture is characterized mainly by altered mental status with memory deficits, ophthalmoparesis, and ataxia, although other clinical manifestations may also be present. The current case presents certain clinical difficulties regarding the diagnosis when confronting an atypical presentation of a classical disease in an acute setting when a decision to administer an intravenous thrombolytic agent needs to be made.
